STREET HEALTH COMMUNITY NURSING FOUNDATION

TORONTO, Ontario, M5A 2A1 Canada

Mission Statement

Street Health provides healthcare for those who are homeless or lack secure housing in the mid-east region of Toronto.

About This Cause

Street Health provides access to physical and mental health services. Our work focuses on the mid-east core of Toronto. We are located in an area that has the largest concentration of shelters and drop-in centre, as well as rooming houses and housing projects. This region has one of the highest rates of EMS calls for drug overdoses and has been deeply impacted by the opioid/fentanyl crisis. Services include: • Nursing (Registered Nurses and Nurse Practitioner), • Community mental health (for individuals with complex personal situations who are unable to obtain services at mainstream organizations, and/or who are barred from other supports), • ID replacement and safe storage (Ontario Health cards, birth certificates, SIN numbers and Record of Landing). Includes a secure mailing address service, • Harm reduction (supporting the health of individuals who use substances and/or engage in sex work). Includes an Overdose Prevention Site (OPS) and a Safer Opioid Supply (SOS) program. We work from our main location, but also have partnerships to support a mobile outreach (health bus), as well as working at COVID-related safe hotels and shelters. Client services provide immediate access to basic personal care and hygiene supplies, as well as health promoting winter warmth/summer relief items and sleeping bags
