Mission Statement

We envision a healthy, active and inclusive community where physical, mental and social well-being is achieved. Stonegate Community Health Centre works to improve the well-being of our community. Together, we identify individual and community needs and respond by providing accessible and inclusive health services and health promotion programs. Guiding Principles: 1. Our work is compassionate, fair and respectful. 2. We support and encourage individuals and the community to set and achieve goals for optimal health and well-being. 3. We engage with the community to address systemic barriers that compromise the health of our community and its individuals. 4. Some members of our community have greater needs and fewer choices so we must be tenacious and responsive in what we do to achieve health equity. 5. We are committed to cultural safety for all clients and will make special effort to ensure Indigenous cultural safety. 6. Greater outcomes are achieved through partnership and collaboration. 7. We invest in our staff and are committed to excellence and sustainability through sound fiscal management, innovation, continuous learning and a culture that values change and adaptability.

About This Cause

Stonegate Community Association is a charitable organization governed by a community based Board of Directors. It operates the Stonegate Community Health Centre (SCHC) that provides comprehensive health and social services to residents living in South-East Etobicoke. We provide comprehensive, co-ordinated, primary health care, illness prevention and health promotion for individuals, families, groups as well as community-level interventions. Our dedicated team of professionals provide coordinated programs and services aimed at maintaining wellness and improving health of people of all ages in our community. We deliver our programs and services with a particular focus on serving the following clients based on need: children and at-risk youth; women; seniors and newcomers – including immigrants and refugees.    We are able to accomplish much in our neighbourhood because we work with individuals, families and the community to strengthen their capacity to better respond to issues affecting their physical, mental and social health and well-being. We do this through: • Primary health care through the practice of family medicine, nursing services, chiropody, nutrition, mental health counseling, social work, diabetes and asthma management programs and a smoking cessation clinic. • Community services including various women’s, youth and children’s programs; early years services, food access and hunger programs, homelessness prevention and seniors health and wellness programs. • Health Promotion through various community development and advocacy projects The Stonegate Community Health Centre envisions an actively engaged neighbourhood that respects diversity, encourages community-based leadership, and strives to achieve its full potential. Our goals include: • Ensuring that programs and services are accessible to all residents of the community. • Striving to remove systemic barriers to health, • Addressing (through both individual and population-based approaches) - the social and environmental conditions that affect the physical, spiritual, emotional, economic and social well-being of our community • Endeavouring to build the capacity of individuals and the community so that they can take control over their own well-being. • Working with individuals in a supportive and educative way as active participants in health and social programs and services. • Being compassionate, fair, and respectful of the diversity of our community residents. • Being accountable to our community
