To demonstrate God’s love through person-centred care services to support the needs of the elderly, disadvantaged individuals and families in Chinatown and beyond.

About This Cause

Yong-en Care Centre (YCC) is a community based, registered charity, and an Institute of Public Character (IPC) that has been serving the community in Singapore, Chinatown, since 1996. In collaboration with other social service agencies and partners, we have improved lives through our areas of expertise that include: - Dementia Care; - Home Care; - Active Ageing; and - Social support for individuals and families with needs through a combination of short to medium term support (i.e. Financial Assistance and Food Distribution – PantryPlenish, BreadWalk, FreshFood); and longer period of intervention through individual or group work and programmes (i.e. Casework, Counselling, Support Groups, Youth Mental Wellness programme, Tuition, Childminding programme and Bursary). Over 900 beneficiaries were served by our programmes and services in year 2023. *Caring for Seniors: 3 core programmes providing comprehensive services, supporting the needs of our senior beneficiaries and creating the continuum of care. 1. Dementia Care Provides centre-based therapeutic programme for persons living with dementia/Alzheimer’s disease. We adopt a person-centred approach, curating meaningful and culturally appropriate activities to allow our clients to age with dignity. • 36 seniors ageing meaningfully. • Rated 91% for Caregiver Satisfaction (Target: 75%) • Maintenance/Improve cognitive status - MMSE score: 81% (Target: 75%) 2. Home Care Provides home-based medical and nursing services, that is affordable and reliant for seniors who are homebound due to health conditions/frailty. Transitional and continuity of medical/nursing needs to seniors discharged from hospitals. Impact: • 146 homebound seniors served. • Client Satisfaction Rate: 96% (Target: 75%) • Hospital re-admission rate: 11% (Target: below 30%) 3. Active-Ageing Programmes: Promotes positive and healthy ageing using an E3 model that focuses on Engaging, Enriching, and Empowering senior participants to stay healthy, and to age meaningfully. • 312 seniors purposefully engaged at Yong-en Active Hub • 65% seniors maintained their physical frailty* score and did not deteriorate after joining Gym Tonic programme. • 27% seniors improved their physical frailty* score after joining the gem tonic programme. * Supporting Families Promote and improve the well-being of each individual in the family to achieve independence, stability, and resilience. Deliver relevant services which provide beneficiaries with ways to overcome the challenges faced in aspiring towards a higher quality of life. Collaborate with partners in the social service ecosystem to support needs in the community. • 185 social work cases • 59 placed on Financial Assistance scheme • 17 mothers in Women of Strength support group. • 148 households and 136 households benefitting from our PantryPlenish and BreadWalk programmes respectively. Programmes incorporate elements of advocacy for healthy eating habits as well as befriending via volunteers for a more sustainable impact. *Nurturing Children and Youths Often, parents of low-income families are overwhelmed with meeting financial and basic needs of their families. A lack of involvement in their children’s academic, socio-emotional and psychological well-being is an issue which needs urgent attention. Yong-en has in place, programmes for our children and youth to directly address them . • 45 students enriched in weekly free Tuition • 50 students encouraged with Yong-en Care Centre Annual Education Awards. • 43 youth meaningfully engaged at Kukoh+ (our newly set up Youth & Children Centre).
