Mission Statement
The Community Family (TCF) is an adult day health program with beautiful, state-of-the-art centers located in Everett, Medford, and Lowell. TCF’s mission is to provide high-quality services and care that keep adults in their homes as long as possible. Our services are based on a deep respect for the dignity, self-esteem, and vitality of those we serve. TCF has kept thousands of frail elders, those with Alzheimer’s disease and disabled adults safe, healthy, and in the community for more than 35 years. Each day, we provide medical care, therapeutic activities, nutrition, and social services to more than 140 individuals at our three centers. TCF serves a diverse group of individuals from a variety of ethnic and cultural backgrounds that reflect the communities served, and the majority of our clients are poor. Our mission and services extend to families, other caregivers and the general public.
About This Cause
The Community Family (TCF) provides high-quality adult day health care to elderly and disabled adults at three centers, located in Everett, Medford and Lowell, MA. All of those attending our centers have at least one chronic medical condition requiring intervention and on-going monitoring. Those attending the Medford and Lowell centers have physical and cognitive deficits caused by Alzheimer’s disease or other forms of dementia. The Everett center serves a large group of adults with multiple medical and mental health diagnoses. The three centers serve in total 250 individuals, with an average of 140 each day. In keeping with our mission, TCF seeks to keep those we care for safely in the community and out of nursing homes as long as possible. Without our care, we estimate that 60% of our participants would be forced to enter a nursing home in order to receive the care they need. TCF develops individualized service plans to meet the unique needs of each client - while also supporting family and caregivers who have made the commitment to care for their loved ones at home. Our programs and services include medical services, rehabilitative therapies and preventive care, therapeutic activities, nutrition (serving meals and snacks), direct care services such as personal care, hairdressing, and bathing, transportation from clients’ homes to our doors and caregiver support. Our outreach and impact extends beyond our doors through active community education, engagement and advocacy. TCF serves a clientele that is ethnically diverse and largely poor. The majority of our clients have annual incomes of less than $25,000, with 60% qualifying financially for subsidized services through Medicaid. For those not meeting these qualifications, we have established a Client Outreach Fund program to assist needy individuals and families with the cost of care. This fund reaches out to families facing particularly challenging circumstances. The COF enables us to reach some of the most disadvantaged individuals and families in the communities we serve that would otherwise fall through the cracks of the healthcare system and go without the care they need and deserve. It also enables us to welcome all individuals into our programs, no matter their ability to pay for the cost of care. The Community Family’s exclusive focus is quality adult day health services, unlike most adult day health programs that are sponsored by larger health or social service organizations. We are, in fact, the only nonprofit provider in the state that owns and operates three modern centers designed solely to provide these services. Over the past 35 years, TCF has provided exceptional leadership and progressive thinking in our field - and in the communities we serve. Among our accomplishments are: • Opening one of the first adult day health programs in Massachusetts; • Opening the first Alzheimer’s center in the nation and becoming Massachusetts’ largest provider of Alzheimer’s day programs; • Building and operating three contemporary, state-of-the-art adult day health centers; • Opening the first green adult day health center in Massachusetts (Everett, in 2010); • Developing internal transportation for greater quality control and increased flexibility to meet the needs of our families; • Initiating innovative programming like an intergenerational Wii-based exercise, Chair Yoga, Laughter Yoga and Alzheimer’s Arts. Our comprehensive services in a home-like environment are a distinguishing characteristic that gives TCF a unique role in the continuum of long-term care services. TCF’s Board of Directors and management team have provided the organization with a strong track record of success. Shrewd oversight and exemplary leadership has enabled the organization to remain fiscally sound despite numerous challenges brought about by stagnant reimbursement rates for our services, rapidly increasing expenses and an overall poor economy. Because of this, we are in a better position than many adult day health providers to navigate through the current difficult fiscal environment. TCF is providing a critical role in the local community and beyond. At our three centers, we are the primary (or only) facility in those geographic areas offering the type of care that we do. Furthermore, we are often assisting underserved or low-income individuals in these communities. On a broader scale, we are fulfilling an immense role in the healthcare industry. At a time when the population is aging and Alzheimer’s disease is becoming more and more prevalent, we are keeping people at home longer and giving respite to caregivers – all while lowering the cost of care. TCF is the best investment society can make to ensure the most cost-efficient means of providing high-quality, long-term care to our elders and disabled.
