At the Community Health Center of Mason City, we are open to all residents regardless of insurance status. We accept most insurance and have a sliding pay scale for those who qualify. Come visit our financial advocate for assistance. 24-hour on-call coverage is available, and we serve seven counties surrounding Fort Dodge as well as Mason City and Dayton. Our caring staff is committed to your well-being and getting you back to feeling like yourself again. At Community Health Center we serve a diverse patient base and see babies into adulthood. We offer medical, dental, and behavioral health services and are open to all regardless of insurance status. We see all people, regardless of age, sexual orientation or gender. We serve patients in multiple languages and have many bilingual Spanish-speaking staff. We accept Medicare, Medicaid, and most private insurance and we have a sliding pay scale for those who qualify. Community Health Center strives to provide health education, promote healthy lifestyles and help all to live the live they want to live. We have social workers available to connect patients to outside resources in the community such as transportation, housing, and food assistance. 24-hour on-call coverage is available, and we serve seven counties surrounding Fort Dodge as well as Mason City, Dayton, and Eagle Grove. Since 1999, we have been offering comprehensive healthcare that includes dental, behavioral health, and general medical assistance for women, children, and men alike. Just some of our services include, but are not limited to the following: Chronic disease management Annual exams Oral health screenings Mental wellness screening/management Acute care Immunizations Well child checks Sports, work, and DOT physicals Each of our locations offers additional outreach services to those in and around the local area. Services include eligibility and enrollment assistance, health education, interpretation/translation, care management services, social worker consultations, referrals to community support services, and referrals to specialists. Mason City Area Outreach Services: Social Worker | Our in-house social worker can assist with community resource referrals to housing, food, transportation, and other emergency assistance programs. Financial Advocate | Community Health Center of Mason City has a trained Certified Application Counselor to review your health insurance needs. Our insurance enrollment services are free and open to the general public as well as CHC patients. In addition, our Financial Advocate can assist you with setting up a payment plan for your healthcare services. Anyone can make an appointment or walk-in during office hours to receive outreach assistance. To make an appointment call 641-450-0601. This assistance is free of charge. Interpretation and Translation Services| We have many interpreters on staff to bridge the gap of language barriers for patients being seen by a provider or in need of information on the phone. Many of our printed materials are also available in multiple languages. Community Outreach Activities | The Community Health Center of Mason City participates in and leads many activities throughout the year within the Mason City and Clear Lake Communities. Our Community Outreach Coordinator represents our services and helps advocate for our patient's needs through participation in civic groups, community leadership, information and resource sharing, and local and federal level healthcare advocacy. Back to School Block Party | Held annually in August during National Health Center Week this event is free and open to the entire North Iowa community. During the event children heading back to school can receive free well-child checks, immunizations, and sports physicals. The United Way of North Central Iowa, the Salvation Army of Mason City, and other local partners assist with the event. The afternoon/evening also includes a free meal, kid's activities, and local resource information for families. Local businesses and organizations can get involved by participating or donating goods, services, or funding to offset the cost of the event. Make It OK Event | In partnership with the Mason City Chamber of Commerce, the City of Mason City, and Cerro Gordo Public Health, the Community Health Center of Mason City leads the organization of a mental health awareness event in North Iowa. The initiative is part of the Iowa Healthiest State Initiative and the Make It OK campaign. By coming together as a community we can help stop the stigmas associated with mental illness by sharing education, debunking assumptions and misconceptions, and raising awareness about the healthcare resources that are available for mental health in North Iowa. Cerro Gordo County mental health resource providers attend the event and set up tables and activities to help the community learn where to go for mental healthcare support for themselves, their loved ones, and neighbors in the community. 5-2-1-0 Initiative | The initiative, which is a community collaboration, will help bring Iowa Department of Public Health funding to Mason City to create Healthier Iowa Kids. CHC has participated in a Healthy Eating Focus Group which accomplished a goal of implementing nutritional prescriptions into our clinic and distributed approximately $5,000 worth of healthy food vouchers to families in our community.
