Mission Statement

Since 1967, NYU LUTHERAN FAMILY HEALTH CENTERS has been serving the ethnically diverse, medically underserved neighborhoods of Southwest and Central Brooklyn, New York and beyond. This Federally Qualified Health Center has evolved into the largest and most comprehensive community health center networks in New York and second largest in the country.

About This Cause

The mission of NYU LUTHERAN FAMILY HEALTH CENTERS is “to improve the overall health of the communities it serves by delivering high quality, culturally-competent primary care and related services within community-based settings.” NYU Lutheran has a proven track record of developing successful community partnerships to create and implement a diverse range of primary care and community service to meet the needs of its patients and improve the total well-being of the neighborhood it serves. As a result, the network is well known and enthusiastically supported by the community. Its governing board, The Sunset Park Health Council Inc., is composed of fifteen members who are either community residents or work in the service area. The composition of membership reflects the ethnic breakdown of the service area population, and includes community leaders from all sectors. The NYU LUTHERAN FAMILY HEALTH CENTERS network generates over 619,000 visits each year, providing services to over 100,000 patients. We are currently certified as a Level-3 Patient Centered Medical Home by the National Committee for Quality Assurance, the highest recognition for providing comprehensive and integrated care centered on the patient. Currently, NYU LUTHERAN FAMILY HEALTH CENTERS operates 9 full-time primary care sites, 15 school based health centers and 16 school dental clinics spanning across 4 New York City boroughs, and co-locates NYC’s largest behavioral health program with HIV primary care services. NYU LUTHERAN FAMILY HEALTH CENTERS offers a full range of comprehensive primary and specialty care services as well as comprehensive dentistry at five health center sites. Complementing this system is a full array of ancillary and diagnostic services, rehabilitation and early intervention programs, two WIC nutrition centers, social work services, and comprehensive health promotion activities including HIV counseling and testing. Our 14-site Community Medicine Program providing needed services to over 5,000 marginalized and homeless populations throughout New York City. NYU LUTHERAN FAMILY HEALTH CENTERS offers a wide scope of enabling services that assist community residents in obtaining the tools for success. Services include financial counseling and assistance in applying for Medicaid and other insurance programs; a sliding fee scale for the uninsured; transportation services; and, language interpreter services. Recognizing that health depends upon an individual’s economic, educational and employment status, NYU Lutheran’s Department of Community Based Programs offers an extensive continuum of family strengthening and community development programs to address the broader needs of the community. Services include adult education; family literacy; youth development; workforce development; case management and supportive services; mental health services; early childhood centers; services for older adults and community service opportunities. Committed to affirming each family's cultural, ethnic and linguistic identities and using community social work practices, our programs are designed to be responsive to evolving family and community issues in the context of a diverse, multicultural society.
