Mission Statement
Peninsula HealthCare Connection (PHC) promotes integrated primary and mental healthcare and intensive case management for homeless individuals or those at risk of becoming homeless in Santa Clara County, regardless of their ability to pay. We will collaborate with local hospitals, healthcare providers, social service agencies, and similarly committed partner agencies to this end.
About This Cause
Peninsula HealthCare Connection (PHC), a Palo Alto based 501(c)(3) nonprofit organization, was founded in 2005 as a collaborative venture between the Community Working Group, Palo Alto Medical Foundation, and Stanford University School of Medicine. Formerly known as Opportunity Health Partners, PHC opened its medical clinic in 2006 at the Opportunity Center in Palo Alto to provide affordable healthcare to underserved and at-risk men and women, particularly individuals experiencing homelessness. In 2011, PHC became a California state licensed clinic, and in 2019 was officially designated as a Federally Qualified Health Center Lookalike (FQHC-LAL). New Directions, a program of PHC since 2013, is an innovative community-based case management program for individuals with complex medical and psychosocial needs. Originally a program of the Healthcare Foundation of Northern and Central California, New Directions was established in 2002 to address the issue of frequent users of emergency departments, and the time, money and inefficient care received by patients in that setting. In 2004 New Directions was chosen to participate in the statewide Frequent Users of Health Services Initiative which demonstrated the efficacy of an intensive case management intervention for reduction of hospital utilization, and linkage to healthcare, behavioral health and other supports and services. Since completion of the Frequent Users of Health Services Initiative, New Directions has partnered with a variety of community and governmental partners to serve some of the most vulnerable members of our community.
