Prevention, Education and Early Intervention Services Parents As Teachers is an early childhood program, for any family with children pre-natal through kindergarten entry, offering home visits, developmental screenings, resource connections, and group activities in the DuBois area, outlying areas of Elk County, and in Jefferson, McKean, and Potter Counties. Nurse Education Services are provided for McKean County families with children birth - 3 with special health, nutrition and care needs. Project RAPPORT offers pregnancy testing, childbirth and pre-natal education, and case management services to pregnant women and teens. Additional services for pregnant and parenting teens promote school attendance, self-esteem, healthy choices and future planning. Short-term prevention services are available for youth at-risk. Services are provided in Clearfield, Jefferson, Cameron, Elk, McKean and Potter Counties. Early Intervention provides individual education activities, speech therapy, physical therapy, occupational therapy, and hearing and vision services for children birth - 3 with a developmental delay, in McKean, Warren, Potter, Cameron, and Elk Counties. The Nurturing Parenting Program serves McKean County families with children ages 5 -17. Sample topics for youth include stress management, choices and consequences, school commitment/truancy, healthy relationships, Internet safety, self-esteem, anger management, peer pressure, and bullying. Family Centers, located in McKean County, offer workshops, play groups, family fun activities, screening events, and group led programs. Children In The Middle assists divorcing parents in maintaining positive communication in an effort to decrease child stress and anxiety. Incredible Years, for parents of children ages 2-8, prevents conduct disorder through interactive parenting, reinforcement skills, child-directed play, and positive discipline techniques. The Strengthening Families Program serves parents and youth ages 10 -14 with a focus on communication, coping skills and peer pressure resistance skills to prevent youth substance use. Big Brothers Big Sisters provides professionally supported one-on-one mentoring services delivered by carefully screened adult volunteers to children ages 5 -18 residing in Jefferson, Elk and McKean Counties. Independent Living provides mentoring and life skills instruction to at-risk youth in McKean County. Support Services Blended Case Management provides assistance to McKean County individuals with a serious mentally illness to help them identify and access available services, community resources, and benefits and to resolve daily living problems. BCM helps to ensure continuity of care and coordinates treatment team communication. BCM is on call 24/7 for crisis intervention, diversion from inpatient care, and support during inpatient admission. Wellness Nurse Services are provided to McKean County clients receiving BCM who are determined to be at risk due to a physical health condition. Services include primary care coordination, wellness education, and consultation. Peer Support is provided by staff with a mental health condition that have achieved stability in recovery. Services are designed to provide mentoring, social support and practical advice to McKean County adults identified as having a serious mental health diagnosis. Services focus on advocacy and empowerment, skill development, wellness, self-esteem, and engagement in use of natural supports and community resources. STEPS Drop In Center offers recreational, social, craft and support activities for adult mental health consumers in McKean County. Mobile Psychiatric Rehabilitation assists McKean County adults who have a serious mental illness to enhance their skills and development in areas of living, wellness, education, vocational and social environments. Supports Coordination provides assistance to persons with intellectual disabilities to assist them in identifying services, community resources, and benefits to meet their needs. Services are delivered in the home and community to McKean County residents. Intervention/ Treatment Services Student Assistance Program services are co-facilitated as part of a multi-disciplinary team with the McKean County School Districts. Assessment, consultation, short-term intervention, and referral services for identified youth. Outpatient Mental Health Treatment is provided at five community clinic locations including Bradford, Kane, Smethport and Port Allegany in McKean County and Coudersport in Potter County. In addition, The Guidance Center operates 12 School Based Outpatient Clinics in the McKean County Schools. Services include counseling, psychological testing and psychiatric care. Telemedicine is utilized to access specialized diagnostic and treatment services for children under the age of five. Individual, family and group counseling is offered. Groups include sex offender treatment, forensic re-entry, grief, and eating disorders. A variety of evidence-based therapies are provided. Outpatient Substance Abuse Treatment is provided at the Bradford clinic in McKean County. Behavioral Health Rehabilitation Services (BHRS) is provided in McKean County for children with serious emotional and behavioral support needs. Behavioral Specialist Consultants design and direct implementation of behavior modification plans in a collaboration with the family and treatment team. Mobile Therapists provide family and individual therapy. Therapeutic Staff Support workers monitor behavior, implement behavior modifications and crisis response plans, and provide one-to-one interventions to assist youth in improving behavior, anger management skills, self-esteem, and social skills. Family Based Services provides intensive support for McKean County families with children who have serious emotional and behavioral disturbances and who are at risk of out-of-home placement due to family conflict and crisis. The service is delivered by a team consisting of a Therapist and a Mental Health Worker. Therapy focuses on improving relationships and family functioning. The Mental Health worker provides case management and support. Crisis Intervention is provided in McKean County including assessment, brief intervention, stabilization, including voluntary and involuntary admission to a psychiatric hospital, consultation, and connections with other community support services. Crisis services include a 24-hour emergency telephone hotline, walk in crisis counseling and psychiatric appointments, and mobile crisis counseling.
