Utah Partners for Health provides compassionate, comprehensive, quality care to empower underserved individuals, families, and communities in a patient centered medical home.
About This Cause
Utah Partners for Health’s mission is: to provide access to primary healthcare services for underserved individuals using an innovative model in partnership with the community. Our passion is to fill gaps in Utah's healthcare system. Utah Partners for Health (UPFH), is a 501(c)(3) charitable organization, established in 2003 by healthcare professionals and community leaders in order to address the growing problems of the underserved in Utah. Our three-model approach is unique; we work with community and medical partners to offer low-cost partnering-clinic care; free mobile clinics for medical and vision care; outreach program; and our Federally Qualified Health Centers: The Mid-Valley Health Clinic providing medical and behavioral health. The UPFH Family Clinic in West Jordan, and the UPFH Dental and Behavioral Health Clinic in Midvale, Utah. Model 1 - Partnering Clinics: We partner with healthcare providers who donate an average of 68% of their customary billing to our patients. This model allows us to leverage our donations to $6.00 in healthcare provided for every $1.00 donated to direct services. Patients make a small co-payment (as if they had insurance) and UPFH funds the remaining 15%. Services include medical care for acute, chronic and mental illnesses, eye care, dental care, physical therapy, lab work, radiology (x-ray, mammography, ultrasound), and counseling. This is a viable and inexpensive option for patients needing primary care services, and often keeps them away from unnecessary emergency room visits. 93% of requests for care can be filled on a same or next day basis. Model 2 - Mobile Clinics: Our Mobile Medical Clinic and Mobile Vision Clinic Programs reach into communities that experience cultural and transportation barriers, in addition to the barriers of being low-income and lacking insurance. Patients receive free exams for acute, chronic, or mental health illnesses. Children and adults receive well checks, and even students can get cleared to play sports with a physical. Patients can be checked for diabetes, hypertension, urinary tract infections, strep throat, anemia, pregnancy, and receive nebulizer treatments for respiratory infections and asthma. Mobile Clinic Program clients typically would have used the emergency room or not sought care due to cost, transportation, or lack of understanding of the healthcare system. There are currently no mobile medical clinics of this sort in operation in Utah. In 2014, we added a second trailer to provide eye exams and access to free or low-cost lenses for students and families in Title 1 schools. Model 3 – Federally Qualified Health Centers providing medical, dental, behavioral health, pharmacy, and vision for all patients regardless of their ability to pay: In 2013, our organization was approached by the Mayor of Midvale on behalf of her city, asking if we would consider opening a health clinic to meet a high need in the community for access to affordable healthcare and a primary care physician. This effort then became our third and newest model, which is a Federally Qualified Health Center. The clinic serves a target population designated as a federal medically underserved, and primary care provider shortage, area. One hundred percent of the target population is considered low to moderate income. In 2016 we added the Utah Partners for Health Behavioral Health clinic based on an internal needs assessment. The Behavioral Health Therapists are integrated with our medical team to provide counseling services at a very low cost for clients with incomes of less than 200% FPL. In 2016 we also added the Utah Partners for Health Dental Clinic to provide much needed access for preventive, restorative, and emergency dental care. In 2019, we opened the Utah Partners for Health Family Clinic in West Jordan based on our need to expand services to meet demand. Also in 2019, we added integrated behavioral health to our Mid-Valley Clinic. In 2021, we expanded our Family clinic in West Jordan to be fully integrated. The clinic now provides medical, behavioral health, optical, pharmacy and case management services.
