About This Cause
We believe everyone has the right to quality health care. In 1997, among growing concerns from local school districts that children entering school were unhealthy and not ready to learn, Neighborhood Outreach Access to Health (NOAH) was established to provide volunteers and a visiting nurse practitioner across the Scottsdale and Paradise Valley unified school districts. These caregivers provided on-the-spot care for children, especially those from Title I schools and families with low-income. A mobile clinic, as well as a brick-and-mortar health center, were established by 2001 expanding NOAH’s outreach in the community. In 2011, NOAH became an independent 501(c)(3) nonprofit organization and obtained federally qualified health center (FQHC) status in 2012, gaining sustainable funding for primary care services from the Health Resources and Services Administration (HRSA). What began as one nurse serving uninsured and underinsured Scottsdale school children in 1997, today delivers high-quality, comprehensive health and social services to over 50,000 Valley residents through six health centers in Scottsdale, Phoenix, Glendale. 89% of our current patients are at or below 200% of the federal poverty level. The need for access to quality healthcare continues to grow, and to meet this need so does NOAH. OUR PROGRAMS: Our model of care puts patients’ needs at the center of attention as we deliver comprehensive, whole-person, life-long care from pediatrics to geriatrics. Medical Doctors, nurse practitioners, and physician associates specializing in family, internal, or general medicine provide primary care to patients of all ages. Medical services include: • Routine Checkups • Prescription Management • Referrals • Lab Services • Chronic Disease Management Pediatrics Providers dedicated to working with children ages 0 to 18 offer pediatric services including: • Well-child Visits (including newborns) • Immunizations • Same-day Appointments • Sports Physicals • Illness or Injury • Referrals • Lab Services Dental Dentists, hygienists, and dental assistants offer dental services at Cholla, Desert Mission, and Palomino Health Centers. Preventative, restorative, and emergency treatments include: • Exams and X-rays • Cleaning • Sealants and Fluoride • Fillings • Extractions • Children’s Dental Nutrition Registered dietitians help establish healthy eating habits as well as provide nutritional guidance and education for patients with diet-related conditions including: • Diabetes • Cardiovascular Disease • Digestive Sensitivities • Weight Management • Food Allergies • Kidney Disease Counseling Our counseling team supports patients’ emotional and mental well-being. Providers can help manage and navigate challenging situations including: • Lifestyle Changes • Stress, Anxiety, and Depression • Grief and Loss • Family Challenges Psychiatry Psychiatric providers assess and diagnose mental health conditions and/or substance use disorders and execute treatment plans including therapy, prescription medication, or a combination of both. Commonly treated conditions include: • Anxiety and Depression • Bipolar Disorder • Addiction • ADHD • PTSD Community Resources Community resource specialists support individuals and families with health and community resources including enrollment assistance for: • AHCCCS and KidsCare (Arizona Medicaid) • Transportation Assistance • Cash Assistance (TANF) • Medicare • SNAP/EBT (Food Stamps) ǎ Health Insurance Marketplace • Sliding Fee Scale for Uninsured Patients Telehealth / Virtual Appointments Telehealth connects you with medical, counseling, psychiatry, community resources, and nutrition services remotely using a phone, cellphone, tablet, laptop, or desktop computer. QuickCare QuickCare, powered by NOAH, offers virtual appointments with providers for urgent, non-emergency medical and wellness needs. Same-day appointment availability allows patients to access care within hours or even minutes. Nurse Triage Line NOAH nurses are just a phone call away and offer immediate, expert recommendations for care based on your symptoms and concerns. BY RECOGNIZING SOCIAL DETERMINATES OF HEALTH, NOAH IS CLOSING THE CARE GAP WITH: Family Resource Kits In the summer of 2023, NOAH will begin distributing over (2,000?) Family Resource Kits, each containing either a storybook or coloring book with crayons (available in English or Spanish), along with a parental resource guide for children ages 0-5 or education on health milestones for ages 6-12. A contest was held among NOAH pediatric patients to name the four unique storybook characters developed by the in-house NOAH Marketing team (below). These kits will also contain nutrition tips and recipe cards, tools for managing stress and anxiety, plus dental education and supplies including a brushing guide, toothbrush, floss, toothpaste. Partners in Prevention (HIV Awareness and Prevention Program) Maricopa County has been named one of 50 jurisdictions identified by the CDC as HIV “Hot Spots,” areas with higher infection rates than the state and national average. NOAH onboarded three providers dedicated to the program linking patients to HIV treatment and preventative care, offering HIV testing as part of routine blood work for every patient. In 2022, the team surpassed our goal and administered over 4,900 HIV tests. Refrigerated Food Lockers In the summer of 2023, NOAH will become the first health center in the nation to install refrigerated food lockers. Sixteen lockers in the lobby of Cholla Health Center in Old Town Scottsdale will enable our patients to leave their medical appointments with fresh, perishable fruits and vegetables. Our goal is to further enhance the bags with shelf stable items and other staples to offer complete, healthy meals for the entire week. POP Health (Population Health Team) Community Resource Specialists (CRS) are available to help patients with their social determinants of health (SdoH), the non-clinical issues that can impact health and wellness. CRSs helps patients with access to healthy foods, transportation to doctor’s appointments, and connections to housing and utility assistance. Workforce Development • Community Health Immersion Program (CHIP) CHIP offers hands-on training and additional support for first-year advanced practice providers like physician associates and nurse practitioners. Six CHIP providers were onboarded in 2022 increasing NOAH’s projected appointment capacity by nearly 4,000 visits per year. • Community Health Workers Community Health Workers were added to the team in 2022 to bridge the gap between patient and provider communication with one-on-one support to motivate patients with chronic diseases to take charge of their health. • AHEAD AZ Advancing Health Equity Addressing Disparities in Arizona (AHEAD-AZ) program launched last May as a pilot created to fast-track existing NOAH employees to the role of certified medical assistants. The six-month program includes over 300 total hours of instruction through online classes and on-site training. All eight participants in our inaugural class completed the program, passed certification exams, and were onboarded as new medical assistants at NOAH health centers in December. The second cohort of students and preceptors are participating from March to August 2023. NOAH accepts AHCCCS and KidsCare (Arizona Medicaid), Medicare, and most insurance plans. Everyone is welcome at NOAH regardless of insurance or financial status. Our Sliding Fee Scale program offers reduced rates for those who qualify based on household income and family size. After-Hours Care We are available 24-hours a day, seven days a week for any urgent matters.
