The purpose of Maple City Health Care Center is: a) to foster healthy community in our neighborhood by providing and promoting affordable, accessible, and integrated quality care, and b) to articulate and promote our experience as a sustainable model.

About This Cause

Maple City Health Care Center is a 34 year experiment in community-based, affordable, high quality health care. The success of our experiment has been recognized nationally. The Health Resources and Services Administration (HRSA) recognizes Maple City Health Care Center as a Federally Qualified Health Center and awards us an annual federal grant. In 2012, the Robert Wood Johnson Foundation’s LEAP project selected MCHCC as one of 31 national exemplar health providers. LEAP sent site visitors to learn about Maple City Health Care Center’s innovations. Maple City then sent members of our staff to share best practices with other exemplars. The project resulted in a national model of care (www.improvingprimarycare.org/). The National Committee for Quality Assurance designated Maple City Health Care Center as a Patient Centered Medical Home -- level three (the highest level possible) . Our 6,000 patients benefit from services that include ambulatory care, wellness care, prenatal care, obstetrics, chronic disease management, counseling, substance abuse treatment, dental care, eye care, dietitian consultation, psychiatry, and orthopedics consultation. We use patient care teams to offer an integrated model of care. Each of our 4 patient care teams includes a physician, nurse practitioners, a nurse care coordinator, a nurse health coach, a behavioral health provider, a dietitian, and medical assistants. The members of each patient care team are co-located in the same work space. Co-location facilitates almost constant communication across disciplines. Each patient care team cares for about 1,300 patients. Each care team has real-time access to on-site staff social workers, Spanish/English interpreters, health insurance navigators. We also offer onsite Medicaid enrollment. About a third of our patients live on an income at or below the federal poverty line. About a third of our patients are unable to obtain health insurance. Two-thirds of our patients identify as Hispanic. Half of our patients speak Spanish. History: In 1988, a group of people living and working in the Northside neighborhood of Goshen Indiana formed a non-profit corporation named Maple City Health Care Center. The new organization hired James Nelson Gingerich, a young family physician who wanted to live in and serve the neighborhood. The mission of the health center was (and still is) to (1) foster healthy community by providing and promoting affordable, and integrated high quality health care and to (2) articulate and promote our experience as a sustainable model. The City of Goshen donated the Northside Fire Station. Neighborhood volunteers renovated the fire station and the new center served our first patient on February 20, 1989. In May 2015, we expanded our service area and opened a second site called Vista Community Health Center. In 2021, we integrated dental care and opened our third site called Maple City Dental.
