About This Cause

Our thoughtfully designed programming and service connections make us a leader in care and education, across Chicago and throughout the country. We provide everything from mental health counseling to access to primary medical care - all services that help clients work toward brighter futures. We help enhance wellness and enrich clients’ golden years with Positively Aging, and we provide a helping hand from four-legged companions through Paws n’ Effect. And, our one-of-a-kind drop-in space for youth provides opportunities to do laundry, use a computer, connect to services and sometimes simply have a safe haven from homelessness and life’s challenges. We are small on the landscape of community health care organizations, yet we deliver a powerful impact - at home in Chicago and across the nation. Read more about TPAN's expansive services below. TPAN’s programs and services—including our HIV-focused publication, Positively Aware (PA)—are created to promote equity, inclusion, respectful collaboration and equal access to care. We believe race, ethnicity, sexual orientation, gender identity, socioeconomic status, disability, or geographic location should never prevent anyone from accessing the information, resources and support they need to obtain quality HIV care and services. To that end, our services and publication are available to anyone impacted by HIV, regardless of their ability to pay. TPAN has been recognized by numerous federal agencies for our expertise in tailoring programs to address inequities with underserved populations – specifically with young men and women of color, LGBTQ youth, and older adults. TPAN serves a diverse community of people who are either living with HIV or vulnerable to HIV and health disparities. Our approach to client care is holistic and our services extend far beyond HIV. TPAN has become known for both on-site and wrap-around services that address many aspects of our clients’ lives (e.g. housing, mental health, substance use) that may prevent them from staying connected to care. TPAN also integrates interdisciplinary programs for our clients, including: case management, which eliminates barriers to care by assisting clients navigating the care system; prevention programs, which help individuals reduce their risk for HIV; a youth drop-in center (the Tea Room) which serves as a gateway to care for young adults many of whom are homeless and/or LGBTQ-identified; and, group and individual mental health services that often incorporate peer support with clinical care. TPAN is also an access point to primary medical care. Hundreds of our clients obtain medical care from an on-site provider with Howard Brown Health, which often provides a same-day appointment and immediate linkage to care for individuals referred by our team. The beneficiaries of TPAN’s core direct services are people living in the Chicago metropolitan area who are affected by HIV and other related health challenges, including hepatitis. Nationally, more than 100,000 individuals and healthcare providers across the U.S. – many in underserved and rural areas – receive the treatment education and HIV-focused peer profiles found in Positively Aware magazine, published six times a year by TPAN. TPAN also leverages partnerships to bring our local expertise to national audiences, as with our collaboration with The Reunion Project (TRP), which this year will provide peer support and education to hundreds of HIV-positive individuals in town hall style summits in cities across the country.
