Mission Statement

St. Joseph’s Neighborhood Center, in the tradition of the Sisters of St. Joseph, seeks to identify and respond to the unique health and social needs of individuals and communities in the greater Rochester region.

About This Cause

SJNC was established in Rochester, NY in 1993 by six members of the Sisters of St. Joseph. The organization separately incorporated as its own independent 501©3 in 2013. SJNC delivers services through a small staff of 20 full & part-time employees & a cadre of over 450 professional & lay volunteers providing more than $300,000 worth of donated time to the organization. SJNC provides “health care for the community, by the community” & is able to leverage its longstanding & constantly developing relationships with community providers in order to offer comprehensive services that are significantly more cost effective than traditional medical sites with higher staffing costs. SJNC maintains a service delivery model that is patient centered, comprehensive in scope, integrated in practice, & equity focused. Services include full primary care, specialty care, hearing & eye health assessments, complementary therapies (massage, acupuncture, chiropractic) & mental health counseling, as well as, social service needs assessment & referrals for insurance eligibility, housing, food, clothing assistance, & transportation. Integration is supported through a customized Electronic Health Record (EHR) that allows providers across disciplines to participate in a patient's whole health plan, including understanding the social determinants of health (SDOH) that affect the patient's ability to access services. SJNC believes that pts get better faster & stay healthier longer through its value-based care model. The health center does not accept private or public insurance reimbursements or seek government funding for services. SJNC prides itself on being “fiercely independent” & treating the “whole person.” This independence has a profound impact on the patient experience, as well as, the provider’s practice of providing care. According to one volunteer advanced practice provider, SJNC “…allows me to practice medicine in the way that I want to. There’s a focus on the patient & doing what’s right as opposed to pushing pts through an assembly line to meet billing goals.” Similarly, one of SJNC’s pts recently stated: “I’m a person here. They care. I saw my other doctor for five minutes, tops. Here, they take their time with me. I don’t know where I’d be without them!”
