Mission Statement

Aspire has brought together the skills and resources of a Community Mental Health Center, a Federally Qualified Health Center, a robust social determinants of health (SDoH) program, and the state’s largest residential recovery service provider, Progress House, into one integrated system. Aspire’s integrated system uses a whole health model that addresses all aspects of health, including primary care, behavioral health, and the SDoH. Aspire is considered an innovative healthcare leader in Indiana, and was asked by the state to be involved in creating the standards for Indiana’s “Integrated Care Entity” certification. Aspire’s whole health network gives Hoosiers in Boone, Hamilton, Madison, and Marion counties access to resources and services that work in tandem to address complex needs and circumstances they may be experiencing. Aspire’s primary care, behavioral health, and SDoH providers all work together to create a well-balanced plan of care that addresses the whole health needs of all clients. Treating all aspects of health together, rather than focusing on one aspect of a person’s wellness, results in better outcomes for the people Aspire serves.

About This Cause

Health disparities cause many low income, at-risk, and disabled people to suffer from easily preventable and treatable illnesses that can have a negative impact on their employment and ability to maintain sufficient housing. The stress and environmental dangers of homelessness can result in both behavioral and medical conditions that compound over time and make it difficult for a person to take care of personal health and daily needs. Mental illness can also provide challenges in socializing appropriately at work, advocating personal rights with landlords and utility companies, or may even result in an inability to work. With assistance and training to address the social determinants of health and programs to support low income and disabled people who require healthcare, Aspire Indiana Health has created a system of care that has effectively helped our community members achieve better health outcomes while simultaneously preparing them to become self-sufficient. Aspire’s whole health model includes primary medical care, behavioral health care, and community programs that address the social determinants of health (SDoH)—all in one setting. Primary medical care services address both acute and chronic health needs and include services such as physical exams, well child exams, immunizations, blood draws, in-office quick tests, infectious disease testing, and medication assisted treatment. Behavioral health care services include crisis, addictions, outpatient, rehabilitation, deaf, school based, and therapy services. Community programs that address the SDoH include employment assistance, housing assistance, legal services, and child advocacy center services. Furthermore, Aspire often collaborates with other charitable organizations in order to provide referrals to services not offered at Aspire, receive referrals to Aspire’s programs for clients in need, and to mutually support one another in comprehensive programming for the betterment of the community. Aspire has a strong working relationship with community leaders, homeless shelters, the Social Security Administration, the Department of Child Services, residential recovery facilities, local employers, local law enforcement and prosecutors, and other community health providers. Partnerships help to ensure that Aspire is able to provide the best possible care for all clients in their moment of greatest need. Aspire also participates in coalitions that help them identify community level needs and strategically develop new programs and services for clients. Aspire has also adopted a “no wrong door” policy. This means people are able to receive all of Aspire’s services no matter how they first encounter Aspire. Whether a person comes to Aspire through employment services, primary medical care services, behavioral health services, or substance abuse prevention and treatment, that person will be able to access everything that Aspire has to offer so they can achieve whole-body wellness.
