About This Cause

Since 1982, Delaware Hospice has provided the highest quality hospice and healthcare services and has served as a trusted community partner in end-of-life education and support. As the only not-for-profit hospice in the state of Delaware, the organization has maintained its mission of meeting the needs of 58,000 hospice patients and their families with the support of the community through donations and volunteer hours. Delaware Hospice is proud to hold accreditation through the Joint Commission, voluntarily adhering to its strict standards of quality care. Delaware Hospice offers a range of care programs which offer help to individuals with serious illness at various stages. Seriously-ill patients, who are not medically appropriate for hospice, find help through the Transitions Program. Transitions clients benefit from visits and phone calls, receive information about community services, and get advice on how to adjust to the new challenges in life. Delaware Hospice’s Palliative Care Program offers relief to seriously-ill patients who are receiving active curative treatment. The program provides assistance to deal with the physical and psychosocial burdens of a serious illness through home visits by a Physician, Nurse Practitioner, and/or Social Worker. The Palliative Care Team works with the treating physician to manage symptoms and assist the patient to identify goals of care and how best to meet them. Upon admission to Delaware Hospice’s Hospice Care Program, a multi-disciplinary care team cooperates with the patient’s physician to develop a care plan which ensures that the patient and family members’ needs are met. Nurses monitor patients closely as often as needed to ensure each patient is comfortable, that symptoms are alleviated, and that family members have been instructed on care. Expert Certified Nursing Assistants provide personal care. Social workers assist in identifying challenges and finding solutions, sharing information about agencies or legal issues, or suggesting ways to relieve the burden of the caregivers. Chaplains bring spiritual support in the faith tradition of each family. Volunteers donate their time to give caregivers a break, run errands or just sit and listen. Delaware Hospice cares for patients in their home, facility, hospital, or at the Delaware Hospice Center. The Delaware Hospice Center offers specialized, 24/7 care to patients in need of critical, short-term symptom management. Sixteen private patient-family suites provide a comfortable, peaceful home-like atmosphere. Guests are welcome day and night and are free to use the spacious gathering rooms including a living room, kitchen and children’s playroom, as well as a community resource library. When a loved one passes, family members begin a journey through grief. Delaware Hospice’s Bereavement Program offers support to adults and children through cards, calls from bereavement counselors, and invitations to grief support groups. The bereavement follow-up continues for sixteen months after loss. The New Hope program is a grief program specifically for children and teens who have suffered a loss. Support is provided through one-on-one counseling, school groups, and our 4 day summer day camp. Delaware Hospice’s standard care and programs are offered at no fee because they are covered by Medicare, Medicaid, and private insurance. As a not-for-profit, Delaware Hospice is supported by generous gifts and contributions from individuals, families, corporations and community organizations. No one is denied care based on a lack of healthcare coverage.
