Community of Hope's mission is to improve health and end family homelessness to make Washington, DC more equitable.

About This Cause

For over four decades, Community of Hope has worked tirelessly to improve health and end family homelessness in our nation’s capital. During that time, we have grown, expanding from one service site in Ward 1 to eight main locations and five partner sites in DC’s Wards 1, 5, and 8 – many of which are communities with a historic lack of resources. Our healthcare, housing, and resource connection services now serve 20,000 people annually. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and supportive services at three health centers, regardless of a patient’s insurance status or ability to pay. Our health centers include Conway Health and Resource Center (SW), Marie Reed Health Center (NW), and Family Health and Birth Center (NE). Our medical services include sick and well visits, perinatal care, chronic disease management, care coordination, and insurance enrollment. At two centers, we offer comprehensive dental services, including free dental care for seniors. At the other center, we operate the only free-standing birth center in DC and a pharmacy available to all our patients. Our comprehensive maternal and child health programs ensure healthy beginnings for hundreds of families every year. We also offer WIC (Women, Infant, Children) nutrition program at three of our sites. At our health centers, community resource hub in Ward 8, and at three schools, we offer a wide range of emotional wellness services. For people experiencing homelessness, we offer programs across the homelessness continuum of care. Programs for families include Prevention, Short-term Housing (i.e., temporary shelter), Rapid Re-Housing, and Permanent Supportive Housing. Whether they are facing an immediate housing crisis or experiencing chronic homelessness, families can access services ranging from case management, youth services, housing search assistance, and employment navigation. For individuals without children, we offer a unique Bridge housing solution and Permanent Supportive Housing in their own apartments. We integrate our work in key ways like connecting people in shelters to emotional wellness therapy or, if needed, to substance use treatment in our health centers. We provide extra support to pregnant people experiencing homelessness and connect teens in the community to our health and well-being programs. Our Bellevue Family Success Center, located at our Ward 8 health center, connects families experiencing hardships with existing government and community resources. Some clients are assisted through a single vital interaction, such as help with accessing healthy food, while others seek case management for help navigating crises. The Center also offers drop-in hours with access to job readiness support, housing resources, parenting classes, and more. Despite poverty and other challenges, the people we serve overcome enormous obstacles to achieve health and wellbeing. Across our core programming areas, we are leading and advocating for system change through our strong relationships with DC government agencies, engagement with our city council, and partnering with other nonprofits through local and national coalitions. With a team of approximately 425 staff, hundreds of volunteers, and thousands of donors, we have built a strong and sustainable organization that advances our vision of ensuring that everyone in Washington, DC has good health, a stable home, family-sustaining income, and a hope-filled future.
