About This Cause
As one of the well-known nonprofit community organizations, the Chinese Culture and Community Service Center (CCACC), after 42 years of development, continues to build the benchmark in all directions, striving to improve the quality and scope of our community The COVID-19 pandemic became a stimulus for our resilient growth, especially in health care service. Based on the report of CCACC Health Center, in FY2024 the number of visits for various healthcare services, consultations, treatments including Covid-19 vaccinations, was up to over 20,000. We have identified one senior who needs more intensive mental health services due to her severe depression. We have referred the senior to our psychiatrist since the client was running out of the medication but could not have an appointment with a psychiatrist. The inability to get her refill medication made her feel more anxious and depressed. We first made an appointment for the client with our clinical social worker, Ms. Lu, to do an initial psychotherapy, and our NP, Ms. Do to see her general health condition and whether we can prescribe her some medication for a short period of time. The client felt much more relaxed after getting her medication and was seen by Dr. Shao, our psychiatrist afterwards. We made a follow up with the client a month later and she reported 14 points decrease in PHQ-9 and 11 points reduction in GAD-7. ------------------------------------------------------------------------------------------------------------------------------- Mr. Xu was a chef with low pay while wife was not able to work due to health conditions. The family has been struggling with daily expenses after Mr. Xu retired. After consulting with the BEC team, the Xu family applied for SNAP to help ease the financial difficulties. They did get approved for the benefit but they never received an EBT card after several tries to report the issue to DHS. When helping Mr. Xu with the SNAP recertification this month, we helped him write a letter reporting the EBT card issue again and submitted the letter together with all the recertification documents. Mr. Xu finally receives his EBT card and has access to the SNAP money. ------------------------------------------------------------------------------------------------------------------------------- Mrs. Fan got approved for SNAP and received the phone notification about her benefit redetermination interview. After multiple tries to contact the office of eligibility and service, we are able to discover that her interview is canceled because she failed to submit the review form and the supporting documents. She has only two days until the deadline to avoid a termination of her benefit. We came up with the plan to prepare all the supporting documents and went to the DHHS office to require a form onsite and
