About This Cause

Wendell Foster’s licensed Intermediate Care Facility (ICF) for individuals with disabilities consists of four eight-bedroom/four-bathroom homes for 63 individuals. Residential, active treatment, and nursing support are offered 24 hours per day. Each person living in the ICF has a person-centered, individualized support plan based on their strengths, interests, and needs. The Intermediate Care Facility is focused on providing each person with the best life experiences possible. Services include recreational services, buffet-style dining, nursing, residential, assistive technology, physical therapy, occupational therapy, and speech therapy. Each individual is evaluated, and an individualized plan is developed to enable those living here to reach their goals and dreams. Through teamwork and a focus on what each person wants out of life, we have achieved what others have considered the impossible. Some of the great experiences that the ICF has achieved over the past few years include skiing in Colorado, as well as vacations to Las Vegas, Disney Orlando, and Panama City Beach, to name a few. We are not just a home but support and family as well. We strive to provide those who live in the ICF with the same life experiences as any other typical family. Outpatient Therapies: Wendell Foster’s Comprehensive Outpatient Rehabilitation Facility (CORF) houses all outpatient therapies, including Physical, Occupational, and Speech Therapies. The therapies include the evaluation and treatment of people of all ages, from birth through adulthood. Services are offered both at our site and within the school setting through contractual arrangements with some area school systems. Depending on the coverage, Medicaid, Medicare, and private insurance may pay for expenses related to the services. Community Living: Wendell Foster Community Living Services offer individualized community-based support to participants who meet ICF/IID criteria. Participants and their teamwork develop supports that ensure individuals are active contributing members of their community. Community Services are offered with respect, designed to ensure participants are afforded choice, and provided in a manner whereby participants feel and are safe in the community. Wendell Foster Community Services provides support to individuals allocated funding through the Supports for Community Living and Michelle P Waivers. Supports can also be provided on a fee-for-service basis. Michelle P Waiver Services are provided for persons with a developmental or intellectual disability who require a protected environment while learning living skills, participating in educational programs, or gaining awareness of their environment. Participants must meet Medicaid financial eligibility requirements. Wendell Foster provides the following Michelle P services: • Case Management • Day Services • Community Living Supports • Respite • Homemaker Service • Personal Care • Attendant Care • Environmental/Minor Home Adaptation Community Living Supports (CLS) are provided to facilitate independence and promote integration into the community. In order to receive CLS, participants must reside in their own home or family’s home. This service is a one-to-one ratio with the focus completely on the participant. Wendell Foster offers accessible housing to adults who need a supported home environment. Supports for Community Living was developed as a Medicaid waiver program for Kentucky residents, serving as an alternative to institutional care for individuals with intellectual and/or developmental disabilities. This program provides 24-hour staffing support while focusing on promoting the independence and maximum potential of each individual. Physical, occupational, and speech therapies, as well as behavioral and psychological services, are available to people living in this setting. Some people receiving Support for Community Living also attend day training programs and recreational activities of their choice. Supports offered include but are not limited to: • Money management • Social interaction • Household tasks • Leisure activities, etc. • Cooking • Housekeeping • Laundry, etc. Personal care is a support option for individuals who receive Michelle P Waiver (MPW) funding. This support is tailored to the participant’s needs. Supports consist of assistance with eating, bathing, dressing, personal hygiene, or other activities of daily living. Day Services are provided by a direct support professional in a non-residential setting and are designed to foster the acquisition of skills, build positive social behavior and interpersonal competence, and foster greater independence and personal choice. Day Services also provide active small group training to prepare a person to transition from school to adult responsibilities, community integration, and work. Day Services are paid for by Michelle P and Support for Community Living waivers, as well as fee-for-service. Respite care services are available for families when emergencies arise, or a well-deserved break is needed. Respite can be provided at Wendell Foster through private pay or may be eligible for the Michelle P Waiver Services. Case Management is an approach to coordinating care, support, and services for a person receiving Michelle P Waiver (MPW) funding using a Person-Centered Planning process. It provides the person and his or her family with a point person/Case Manager who assists in connecting with community and waiver resources. The Case Manager works closely with the person to ensure ongoing satisfaction, that needs are met, and that health, safety, and welfare assurances are in place. The Case Manager is to serve as an advocate to safeguard the person’s choices among many potential providers. Case management services are typically paid for by Supports for Community Living (SCL) and MPW waivers. Services can also be provided on a fee-for-service basis. Technology & Resources - Assistive Technology: Assistive Technology (AT) is defined as “any item, piece of equipment or product system, whether acquired commercially off the shelf, modified, or customized, that is used to increase, maintain, or improve the functional capabilities of individuals with disabilities.” AT includes communication devices, adapted computer equipment, switches, environmental controls, seating, positioning, mobility equipment, amplified phones, desktop, portable magnifiers, etc. Wendell Foster is a participating member of the Kentucky Assistive Technology Services (KATS) Network, whose mission is to make AT information, devices, and services easily obtainable throughout the state. We offer the following services: • Device Demonstrations – Enables consumers to compare the features and benefits of similar devices to aid in decision making. • Lending Library – Allows consumers to loan items on a trial basis to assist in decision making, to serve as a loaner while a device is being replaced, or to provide short-term accommodation. • Training – We offer informal and professional training designed to increase knowledge and skills regarding AT. • Reutilization Program – Assist consumers in obtaining used equipment at no cost. Community Resources We offer community resources for people with disabilities, families, educators, health care, and service providers. • Staff can provide information and assistance on a variety of disability topics and help navigate local and state resources. • We provide learning opportunities throughout the year for families, professionals, and educators Support Programs • Wendell Foster offers several community support programs to assist you in achieving a full engaging life, such as; Person-Centered Training • Family Support • Transportation • Respect Starts Now • Aktion Club • Sibshops
