About This Cause
Since 1985, Albuquerque Health Care for the Homeless (AHCH) has been the premier health and service provider for our neighbors experiencing homelessness. AHCH continues to provide a distinctive continuum of wrap-around supportive services and integrated care teams to address the health-related causes and consequences of homelessness. Designed as a Patient-Centered Medical Home, AHCH meets the complex needs of people experiencing homelessness through both field-based outreach and low-barrier site-based services to create a variety of alternative entryways to care. Our center provides high-quality integrated health services that are trauma-informed and healing-focused, with an emphasis on harm reduction and housing firs.t. Our philosophies of care are rooted in evidence-based models informed by personal choice, dignity, and respect. Last year, over 5,000 people received services from AHCH. Encounters, contacts, visits, across all AHCH services were roughly estimated to be around 15,000. Some of our services include: Primary Health Care, Primary Dental Care, Housing Navigation, Social Supportive Services, Case Management, Behavioral Health Counseling, ArtStreet and Art Therapy, Resource linkages, Shower and Community services, Harm Reduction and Overdose Prevention, and Advocacy. After the pandemic, AHCH strived to reduce barriers for those living on the street and were able to make almost all of our services portable and available on outreach. This month we released our Medical Outreach Van which will be able to provide a full suite of primary medical and dental care to those who cannot travel to our main clinic.
