The Rogue Valley Dialysis Patient Trust Fund is a private, non-profit organization providing confidential emergency assistance to dialysis patients living in So. Oregon and No. California.
About This Cause
WHAT IS THE PATIENT TRUST FUND? The Patient Trust Fund was incorporated in 1989 with the goal of assisting those who not only have experienced the physical effects of kidney failure, but also the emotional and financial stress related to this major illness. The Fund is administered by a volunteer board of directors. Grant applications may be submitted to the Social Services Dept. of Rogue Valley Dialysis Services in Medford, OR or Redwood Dialysis / GP II Dialysis Services in Grants Pass, OR. TYPES OF ASSISTANCE Patients of Rogue Valley, Redwood or GP II Dialysis Services are eligible for grants to assist with medications, transportation, food / nutritional supplements, emergency housing, utilities and other emergencies. The Patient Trust Fund is generally used as a last resort. Our Social Workers will often refer individuals to a variety of other resources for assistance as needed. WHO IS ELIGIBLE? All dialysis patients and recent transplant patients in need who receive dialysis from Rogue Valley, Siskiyou, Redwood, GP II or Klamath Falls Dialysis Services are eligible for assistance. Generally recipients area asked to supply a written estimate or receipt for services. Applicants are screened for financial need and assistance is always limited to the availability of funds. CONTRIBUTIONS Donations to the Fund are greatly needed and sincerely appreciated. Your generosity makes a difference in the quality of life for those in need. There are no staff or management fees and administrative costs are minimal. Thank you for your consideration and kindness in donating to our Fund.
