Momentum for Mental Health exists to help individuals achieve mental and emotional health, discover and reach their potential, and fully participate in life.
About This Cause
Momentum for Mental Health offers a comprehensive treatment, rehabilitation, and housing programs and services to people of all ages recovering from mental illnesses. Specialized culturally-competent services are available for Latino, Asian and African American populations. Momentum provides services at 10 sites and has 15 supported housing facilities. Below are the different programs Momentum offers: Assessment and Customer Service ~ Assessment and Customer Service receives, assesses and tracks referrals of new clients and those that have been closed to Momentum for six months or longer. Outpatient Services ~ This program provides services and crisis intervention for persons with persistent and severe mental illness. Services are provided throughout Santa Clara County and include helping consumers access needed resources and other services in the community. The outpatient service team consists of a variety of individuals that participate in the assessment, planning, and/or implementation of treatment plan. While CSS is the “point person” for clients, there are also other personnel who provide client support when needed. Using a team approach, the CSS, doctors, clinicians, managers and others, work together as a team to endure clients received all the necessary services required to achieve a successful recovery. Transitional Age Youth (TAY) Services ~ TAY Services Program is an intensive outpatient program designed to stabilize crisis situations and resolves behaviors that interfere with an adolescent’s ability to function at home, school, foster care, and/or in a group home. This program is for consumers ranging from ages 14 through 21. Full Service Partnership (FSP) Programs ~ FSP services are based on a recovery-oriented social rehabilitation model. Highly interactive, intensive assistance is provided for small caseloads of consumers with multiple barriers. Momentum’s FSP Adult and Adolescent programs are located at The Alameda and White Road campuses. Rehabilitation Services ~ The Day Rehabilitation programs focuses on skill development, peer support, socialization and life management of illness education. Services are for individuals who need to develop skills to live successful lives, and are provided in structured groups, drop-in centers, and through other self-help opportunities with a focus on empowerment and recovery. Programs are located at Crossroads’ Nikki’s Place on White Road, Donna’s Place at The Alameda and the Discovery Center in Palo Alto campuses. Employment Services ~ Momentum Employment Services, funded by the Department of Rehabilitation, offers a full range employment services to clients who desire to return to work. Services include identifying client interests and abilities, employment preparation, job search skills, placement in training and/or employment to match clients’ skills and interests, and post-employment support. Momentum has full-time Job Developers working in the community to establish relationships with employers in order to generate a variety of opportunities consistent with clients’ needs and employment goals. Job Developers participate in employer education and attend employment interviews with clients as needed, in addition to assisting clients with identified reasonable accommodation and disability disclosure. Momentum provides clients with ongoing support and training after job placement to ensure job retention. Housing Services ~ The Momentum Housing Support Staff assist and support clients to obtain, maintain and retain their housing by teaching the necessary skills for independent living. There and numerous units and some offer subsidized rent at some locations. Other housing services include homeless outreach, referrals, advocacy and emergency housing placements for homeless persons. Crossroads Village ~Residential care homes, also known as "board and care" facilities, provide a long term supervised home environment. The integrated day rehabilitation program assists individuals in increasing independent living skills. Residents receive assistance with medication compliance, meals and activities of daily living. Through a contractual relationship with Santa Clara County Mental Health Department, Momentum provides services throughout Silicon Valley for eligible individuals. Crossroads Village ~ Although Crossroads is a licensed board and care home it is very different from most board and cares. Providing a welcoming, homelike atmosphere for up to 45 residents, Crossroads also includes a wellness focused Day Rehabilitation Program and a team of community integration specialists called The Bridge Team. Day Rehab and Bridge Team assist residents with building skills and making connections to facilitate their return to the community. Residents receive assistance with medication adherence, meals, and social activities. Residential Treatment ~ Momentum’s three Social Rehabilitation Crisis Residential facilities provide group and individual treatment to clients needing support during difficult times. They are SART, FSP Residential and Litteral House in San Jose. and La Selva (is Transitional), a private pay facility in Palo Alto. These programs provide 24-hour residential treatment for adults and is an alternative to admissions or continued placement in an acute or long-term psychiatric facility. Services include psychiatric evaluation and treatment, psychoeducational groups, individual and group counseling, life management skills training, medication stabilization, internal case management, community resources and socialization activities. Outpatient Clinic ~ Psychiatric evaluations and medications are reviewed by psychiatrists. Psychiatric Technicians monitor blood pressure and give injections. Benefits Counseling and Coordination ~ This service assists clients with the often overwhelming task of keeping up with changes in public benefits and helps clients apply for any benefits they may be eligible for. This service is accessed through Assessment & Customer Service at The Alameda campus. Adult Family Services ~ This service provides counseling and education to families of clients. “Family Nights” provide education to Vietnamese, Cambodian, Latino and African-American families. Groups are coordinated by staff. Consumer Advisory Board ~ A board made up of clients currently receiving services from the agency meets monthly with the CEO to bring client concerns to management. Their contributions are taken into account when making long range planning for the agency. Family Advisory Board ~ Comprised of families members of clients currently receiving services from the agency, the Advisory Board meets monthly with the CEO to bring the concerns of clients and family members to the attention of the agency. The Board also contribute to the ideating of services which may affect family members. Rise Above Stigma Panel ~ Rise Above Stigma consists of a panel of speakers that provide public education presentations about their journey to recovery from mental illness. Audience members gain insight and knowledge by listening to the personal stories and views of panel members and families. Since 1997, panelists have presented the journey to audiences all over Santa Clara County. Presentations have been given to large corporations, middle and high schools, churches, mental health programs, community services programs, libraries, and hospitals.
