Pathways' mission is to provide high-quality patient and family-centered home health, hospice and palliative care in the home with kindness, promoting comfort, independence and dignity.
About This Cause
Pathways aims to bring compassion and comfort to people in our community who are restricted to their home because they are recovering from illness or surgery, living with chronic illness, or facing the end of life. Our goal is to provide high quality patient care, while responsibly stewarding organization and philanthropic resources. Pathways is working to improve patient care by focusing on solutions that address specific health issues facing our patients and the community, such as an aging population, more people living with chronic conditions, and an increased demand on hospital emergency rooms. Pathways has initiated several evidence-based programs to address these important issues, including chronic disease management, hospital-to-home transitions, and telemonitoring. These programs enhance patient outcomes by strengthening partnerships with other healthcare providers and adopting technologies to enhance patient outcomes. Pathways has over 42 years of experience providing hospice and home health care in the Bay Area. Our staff provides the framework of excellence, bringing a full spectrum of health care expertise to our patients. Pathways has received the HomeCare Elite designation from the National Research Corporation, awarded to home health agencies that excel in quality patient care. Pathways has also earned The Joint Commission’s Gold Seal of Approval, demonstrating compliance with The Joint Commission’s national standards for health care quality and safety.
