Mission Statement

Center for Elders’ Independence (CEI) is a non-profit Program of All-inclusive Care for the Elderly (PACE) health plan for seniors age 55 and older with chronic and complex medical needs who prefer not to move into a nursing home but whose health problems make it impossible for them to stay at home without the help of caregivers.

About This Cause

CEI's PACE plan offers an alternative to those seniors who prefer to remain in their own home, but whose medical problems make it impossible for them to live at home without assistance from doctors, nurses, social workers, and other caregivers. Personalized health care is planned and provided by an interdisciplinary team of professional providers working together with each participant. When necessary, services may also be provided in your home, in a board and care or other group housing, in a specialist’s office, or in a nursing home and hospital care as well. CEI's Interdisciplinary Care team is composed of a primary care physician, PACE Center manager, registered nurses, physical, occupational and recreational therapists, social worker, dietitian, your home care geriatric aide, PACE Center aides, and transportation representative. As needed, the care team mobilizes other services including medical specialists, laboratory and other diagnostic tests, hospital and nursing home care. Each participant receives an in-depth assessment fby the entire care team upon enrollment and every six months or when there is a change of condition or a request for services; participants play an important role in managing their health and independence. With rigorous monitoring and frequent coordination frail seniors can stay as independent as possible in the place they call home.
