Mission Statement

The Well Community Collective's vision is to improve health and wellness for all youth in rural and marginalized communities through equitable and culturally sensitive youth-centred spaces. The Well CC's mission is to engage rural and marginalized youth and communities to co-develop, co-design, and co-lead the development of low-barrier, stigma-free, integrated care pathways to support timely access to evidence-informed mental health and substance use services to address the social determinants of health. Basically, The Well CC is creating integrated youth wellness (mental, physical, and substance use health spaces co-locating with recreation, education, employment and other social service to support youth (ages 12 -25) with equitable service locations throughout Huron, Perth, Grey and Bruce Counties.

About This Cause

The Well CC is working to develop an Integrated Youth Service (IYS) model for the region of Huron, Perth, Grey and Bruce Counties in response to significant need. The Well CC IYS initiative aligns closely with the YWHO (Youth Wellness Hub Ontario) initiative, and aims to co-design 9 One-Stop Shop Youth Wellness Hubs centred around the needs of the local youth to provide low-barrier stigma free and culturally relevant and responsive care. The Well CC IYS, has created Governance Tables to partner with those working across the system, sharing information and resources openly, building mutually advantageous partnerships, and operating in all ways to build an accessible one-stop shop service model. The Well CC IYS has created robust Youth Action and Family and Community Advisory Councils that have empowered youth to embed their feedback and voices at all levels. Most recently, The Well CC has created a Service Pathways table to begin planning service integration and youth-centric service pathways that embed measurement-based care platforms and ensure all programming works from a harm reduction, trauma-informed, concurrent disorders informed and youth-centered approach. The Well CC IYSN Governance Tables have co-designed the following values and commitments that will guide our collaborative work: ✓ Equity, diversity, inclusion, anti-racism, and belonging ✓ Safer spaces that support trauma-informed, anti-oppressive, harm-reduction and culturally safe practices ✓ Commitment to change ✓ Transparency, openness and respectful communication ✓ Continued collaboration with additional agencies, youth and families ✓ Evidence-based practices and measurement-based outcomes for youth ✓ Low-barrier, stigma-free, equitable access to services for all youth ✓ Youth engagement and leadership with a focus on youth co-design ✓ Collective decision making ✓ Youth-focused care pathways to address mental and substance use health and social supports ✓ An asset-based approach focusing our rural community strengths and opportunities ✓ Commitment to Truth and Reconciliation Calls to Action We know there are potentially effective treatments for youth experiencing mental health and substance use challenges, however, the lack of early and affordable access to these services remains a barrier to overall youth well-being. We have a costly system that operates in crisis. Delivery of youth mental health services through emergency departments is costly and does not provide an appropriate setting of care for most youth. Having IYSN models in place can reduce the economic burden on the mental health and addiction system and can support the diversion from costly system services. The integrated approach of one-stop shop services delivered as hubs by The Well Community Collective responds to youth needs across multiple domains. Having an integrated approach to services fundamentally enhances engagement thereby increasing the effectiveness of mental health treatment. We need upstream and innovative solutions to a system that is not meeting the needs of youth and is often responding when it is too late.
