Mission Statement

Our Mission: Davenport-Perth Neighbourhood and Community Health Centre support people in its neighbourhood, especially those who face economic and/or social barriers, to enrich their lives and the life of our community. We do this by working in partnership with local residents and organizations to deliver a range of community, health, and social support services that are responsive to local needs and opportunities. Our Vision: We envision healthy neighbourhoods where people work together to foster respect and mutual support, celebrate diversity and work collaboratively to ensure a safe, caring, just society for all. Our Values: • Well-being. We believe that all people have the right to the fundamental conditions that enable good health, and the ability to contribute to their full potential. These conditions include: living free from violence, safe affordable housing, liveable incomes, nutritious food, a vibrant eco-system, and access to responsive institutions and high-quality community resources. • Empowerment. About This Cause

Davenport-Perth Neighbourhood and Community Health Centre (DPNCHC) is dedicated to the development of our community and its people. We support people in our neighbourhood, especially those who face economic and/or social barriers, to enrich their lives and the life of our community. We do this by working in partnership with local residents and organizations to deliver a range of community, health and social support services that are responsive to local needs and opportunities. Our core values are equity and empowerment, and we strongly believe in each person’s capacity to take responsibility for their own wellbeing. To this end, all of our programs and services at are created in partnership with those we serve. Our Health Centre offers access to our dedicated team of physicians, nurse practitioners, counsellor/therapists, registered nurses, and a community dietician who work together to provide residents of our community with regular, dependable, and client-centred medical care. Members of our health team share a holistic view of health and wellbeing and strive to take into account each individual's physical, mental, social, and cultural needs when providing care. Our providers demonstrate respect for their patients’ values, preferences and choices regarding their own treatment, and seek the participation of their patients’ family and friends where possible. This approach reflects our recognition that health is not merely determined by biological factors, but by a wide variety of social, economic and environmental influences. Our Community Support Services Programs also address health and social issues by focusing on determinants of health such as housing, social and family support, food access, education and poverty. Through a range of programs and services that are responsive, accountable and accommodating to their unique needs, children, adults, seniors and families in our community can develop skills and share their strengths in an inclusive, supportive environment. Our Program Goals and Objectives: • To provide innovative programs and services that are informed by and responsive to the people we serve; • To provide these programs and health care services in a way that reflects and is sensitive to the multi-cultural, multi-racial composition of the community; • To monitor emerging needs and issues in the community and respond efficiently, effectively and creatively; • To advocate for programs and other appropriate responses where needs and issues have been identified; • To provide a focal point for community involvement in primary health care. • To design health promotion activities that encourage self-help and social responsibility directed at maximizing individual/community well-being. • To improve access to primary health care services and preventative health care services for persons who cannot access these services due to language, culture, economic barriers and barriers of distance and inconvenience; • To document and evaluate program activities to assure their value to the community and demonstrate their benefits. • To demonstrate a collaborative model of multi-service delivery through partnerships with health care professionals, community agencies, faith groups, and other appropriate organizations, who assist in determining the focus, priorities and methods of administering and delivering our primary health care services and community programs. • Ultimately, to empower our clients, offering them a safe path to self-sufficiency and supporting them as they take control of their lives, their health and their community.
