About This Cause

Founded in 2009, Swasth Foundation’s vision is “health and joy for all”. Through our two initiatives, we aim to nurture the well-being of an important target audience by offering high quality, accountability and affordability within our services. Our journey and these two initiatives have been highlighted below: Problem Statement: In India, every year, 1 in 20 people (~60 million Indians) fall into a cycle of impoverishment because of a health shock. Health shocks, defined as unpredictable illnesses that diminish health status have a two-fold impact on one’s financial status through both the payments for medical treatment and the income loss from an inability to work. This forces many in India to adopt sub-optimal strategies of coping with this unprecedented financial burden, especially in the absence of a strong and well implemented public health insurance policy leading to a high proportion of out of pocket expenditure on health (65-80%). These include forgoing income, using savings, informal borrowing, using loans or mortgages, and selling assets and livestock. If a household head falls ill, family members previously not working may begin to do so to substitute for lost income and repay loans, sometimes dropping out of schools and higher educational institutes. This leads to a magnified negative impact of a health event as it feeds effectively into other forms of inequity. Based on a pilot that we ran with the International Labour Organization in rural Maharashtra, we observed that high quality primary care can reduce the incidence of such health shocks by half. Swasth India Medical Centers: This is why we have spent the last 10 years building a network of ‘one-stop-shop’ primary healthcare centres called Swasth India Medical Centers that provide patient-centric, high-quality and accountable care without any fee to the low income households of Mumbai. These medical centers provide general physician consultation, pathology lab services and medicines to low-income communities residing in Mumbai’s slums, communities who experience an extremely high disease burden due to overcrowding, poor hygiene and sanitation facilities and the absence of basic civic amenities. In the last 10-years, we have been successful in creating the most innovative solutions based on a holistic set of values to support our communities in living a healthy life. Over the last 12 years, through innovations like (1) Task shifting by training primary care providers to manage chronic conditions like hypertension and diabetes, (2) Develop a patient adherence program for hypertension and diabetes, and (3) 40% reduction in cost of delivery of care, we have (i) More than doubled the adherence and in-control rates for hypertension and diabetes patients from ~25% to ~60%, (ii) Reduced out-of-pocket expenditure for patients to 10% from 70%, and (iii) Re-institutionalized trust and balance in the relationship between a medical service provider and a patient, by maintaining a patient satisfaction of 4.5+ on a scale of 5. Swasth Yog Institute In the process of providing of primary care facilities to the communities that we serve, we realized the high burden of non-communicable diseases in urban India, especially Mumbai. At this stage we pivoted from creating only a healthcare ecosystem to a health focused ecosystem. Towards this purpose, we first launched the Swasth Yog Institute, an institute that aims to build a cadre of professionally certified health coaches focused at restoring the original state of well-being in all. The curriculum of the courses is developed through research centered on integrated care that harmonizes the knowledge of the west with the wisdom of the east. Our Impact: In the last 10-years, we have been successful in creating and implementing extremely innovative solutions with the aim of reducing out-of-pocket expenditure and re-institutionalizing trust and balance in the relationship between a medical service provider and a patient. Till date, we have served over 1.5 million patients delivering direct patient savings of Rs 250+ Million with a customer satisfaction rating of 4.5 and a process quality compliance of 95%+. Towards the goal of preventing and controlling NCDs, we have screened over 40,000 people in the community and today we treat a portfolio of over 5,500 people for chronic conditions with ~60% of them having their parameters in control. We wish to magnify this impact by catering to a larger patient and community base.
