About This Cause

1. ORGANIZATIONAL INFORMATION Mangu dispensary is a Faith Based Health Facility started in the 1940s by the Franciscan Missionary sisters of Africa under the auspices of the Catholic Archdiocese of Nairobi. This was in response to the community health needs including maternal and child care services by then. The sisters worked very hard in educating people in the communities on health behaviors and at the same time encouraging expectant mothers to attend antenatal and maternal care services from the dispensary so as to eradicate home deliveries and reduce maternal and infant mortality rates which was a big challenge in the area by then. Mangu dispensary is situated close to Mangu trading center (opposite St Francis Girls High school) in Gatundu North Sub County along Thika-Naivasha road. The facility is currently run and managed by Assumption Sister of Nairobi (ASN). The dispensary is registered under the Kenya Medical Practitioners and Dentists Board, the body responsible for the oversight of all medical practices and institutions in Kenya. The facility has been in existence for more than 60 years today and has had a great impact the communities around as far as health needs are concerned. The current state of the facility in regards to size and capacity can no longer meet the community health needs thus limited to offering general outpatient services. Currently the services offered include; dental healthcare services, basic Laboratory services, Pharmacy services, MCH/PMTCT/ with a comprehensive Care Clinic (CCC) serving persons living with HIV/AIDS. In addition to the above services, Mangu dispensary runs a community outreach Programme by the name Integrated AIDS Programme (IAP) which has for more than 20 years been serving the entire Gatundu North Sub-county offering a variety of services including home based care to bed ridden patients, advocacy and HIV testing services, support for OVCs/guardians and carrying out behavior change education sessions among youths in schools and out of schools. 2. PROJECT LOCATION Gatundu North constituency is one of the political constituency in Kiambu County, Kenya, represented by a member of parliament in the National Assembly. The area has eight administrative locations which are Chania, Gituamba, Gakoe, Kairi, Mangu, Karure, Makwa and Githobokoni. There are 22 sub locations with the main mode of transport in the interior being bodaboda (motorbikes) while vehicles are only available along the Thika-Mangu-Naivasha high way. As management we have a plan to improve and upgrade Mangu Dispensary into a Hospital. This proposed project will be located within the same compound, in Mangu location (politically referred to as Mangu ward) which is one among the eight locations within Gatundu North Sub County. The project will serve the three locations which include Mangu, Karure and Githobokoni comprising of 50% of the sub county population and with no other public health facility apart from Mangu dispensary. Mangu location is approximated 58.9 square kilometers (Kiambu District Planning Unit 2011). The National government is represented by an area Chief while the county government is represented by a Member of County Assembly (MCA) Within Mangu area, the past political leadership did not focus much on improvement of health infrastructure thus the disparity of health facilities distribution. However the trend has now changed and the new political leadership is development conscious and supportive of the mission hospital idea making the environment of our envisioned project more conducive. Population According to KCIDP 2013-2017 the population of Gatundu North sub county was projected at 127000 people in 2017 (51% female, 49% male). Mangu location is the largest in Gatundu North Sub County with a projected population of 26000 people in 2017 representing 22% of the sub county population and with an annual growth rate of 1.6%. The population density was projected at 441pp/kilometer squared in 2017. The Bantu tribes comprise of about 80% of the population with 20% composed of tribes from other parts of Kenya. (Kiambu District Planning Unit 2011) The population composition in Mangu location alone stands at 28% women of child bearing age (15-49) translating to 7280 women, children under 5years at 12.6% (6350pp) and pregnant women at 3% (3810pp). (Gatundu North Health Records and Information office-Igegania level 4 hospital). The average household occupancy is 5pp/hse. Education Gatundu North Sub County has 81 primary schools (58 public, 23private), 39 secondary schools (37public, 2private), 3 polytechnics and only 1 private college with no university (Sub county education office). The total enrolment into primary schools is 20,234 (10,286 Boys, 9,977 Girls), and 11958 (5,675 Boys, 6283 Girls) in secondary schools. From above data it is clear that tertiary education is a big challenge as there is no public college in the sub county thus student have to seek such education from the neighbouring sub counties. Due to the above challenge most students end up not accessing college education because it becomes more costly for the families. Illiteracy levels and school dropouts are higher among women with over 60% of them not being able to go through secondary education and over 70% of those who make it to secondary school unable to access college education (IAP survey). Education is a key external factor affecting health, an educated community is a healthy community, with most women in the area unable to access good education and considering that they have more responsibility bringing up children, health challenges are bound to increase. Putting up the proposed hospital project would help mitigate the expected effects of the situation. Social-economic condition Generally, people live in homesteads due to high land subdivisions and the traditions of the rural setting. People come together to form welfare groups for economic progress. The common economic activities are coffee farming, dairy farming, poultry farming and subsistence farming with an estimated average household income of Ksh 4000-8000 per month with 20% living below a dollar per day (Kiambu District Planning Unit 2011) Unemployment rate in the area is also alarming. The Kenya Nation Bureau of Statistics (KNBS) puts the rate in Kiambu County at 17% (kiambu.go.ke - Unemployment and other sources of income). When translated to Mangu location this means that 4420 of labour force is unemployed. Alcoholism, drug abuse and prostitution are other very common health and social challenges affecting both men and women in the area. This has pushed the governor to start a rehabilitation Programme named “kaa sober” where victims are made busy through casual labour, talent development and in-house rehab for those seriously affected. Domestic water is sourced from bore holes, streams, or piped according to different parts of the location. Electricity is generally available for those who are able to connect from the national grid. The current facility is quite small, squeezed and poorly ventilated which stands in a 50 feet x 50 feet area which contains four rooms, store, and a waiting bay. The space has further been subdivided into small rooms to include a laboratory, dental unit, HTS, pharmacy, consultation room, data room, cash office and CCC room. A few months ago we, applied for funding from the Japanese Embassy Kenya who has gladly accepted to support the construction of a better out-patient structure which is almost complete 6. THE PROJECT GENERAL OBJECTIVES VISION: A healthy and empowered community. Main objectives: The main objectives of Mangu dispensary, a long term plan as stated in the strategic plan are; 1. Upgrading Mangu dispensary into a full pledged mission hospital through infrastructural expansion and purchase of equipment. This will include a spacious out-patient department, MCH & maternity wing, radiography, physiotherapy, ICU, and theatres. 2. To contribute towards the acceleration of the achievement of universal health coverage and well-being of the people Gatundu North sub county and its environs. Project description and goals Mangu dispensary belongs to the Archdiocese of Nairobi and is currently in the process of transformation and growth into a mission hospital envisioned to have a bed capacity of 160. (1) Project Strategic goal To contribute towards the acceleration of the achievement of universal health coverage and well-being of the people Gatundu North sub county and its environs. (2) Project Specific objective To achieve increased capacity of Mangu dispensary to offer expanded health care services through expansion and modernization of the facility into a mission hospital by constructing and equipping of maternity.
