Improve the quality of healthcare for children with heart disease in Mexico
About This Cause
Kardias is a non profit organization dedicated to developing programs of excellence for the healthcare of children with heart disease, as well as promoting education and dissemination of the relevance of congenital heart disease. We want to ensure that Mexican girls and boys with heart disease have access to the medical care they deserve, but what are the components of a program of excellence and how has Kardias managed to integrate them into its program? 1. Volume For a program to be successful, it needs to receive a significant number of cases that allow the medical team to gain experience and present results that can be statistically evaluated. The Kardias Program is based on a unique care model in Mexico, which combines the referral of patients from the public sector and the care of patients from the private sector. Our program has achieved a high annual volume of patients, which has allowed us to create a unit of excellence, with highly trained medical and nursing staff. 2. Dedicated medical team The Kardias program has focused on designing strategies to have a unique multidisciplinary team of health professionals in its cardiology and cardiovascular surgery unit. The creation of a team with these characteristics, consisting of surgeons, cardiologists, anesthesiologists, intensivists, nurses and psychologists dedicated full-time to the program, has allowed us to develop care protocols for girls and boys with congenital heart disease that place us level of the best centers of medical excellence in the world. 3. Complexity For the classification of cases, the risk stratification method RACHS 1 Score is used, based on a medical consensus. This methodology is grouped into six different categories of cardiovascular surgery, from lower to higher risk, which allows statistical analysis to improve the quality of care. In our program, the number of complex surgeries categorized as RACHS-1 four, five and six has gradually increased, without sacrificing results or quality. 4. Infrastructure. In high specialty centers it is essential to have cutting-edge technology. The Kardias program operates in two centers and both have the infrastructure and equipment necessary to perform surgeries of the highest complexity with the necessary quality. Both infrastructure and equipment must receive the necessary maintenance constantly to maintain the level of excellence of the program. In the Kardias Program, each of the centers of the program has a Pediatric Cardiovascular Intensive Care Unit (UCICV) and both are managed by an interdisciplinary team of health professionals. 5. Commitment to transparency The Kardias Program is the only one in Mexico that has audited surgical results thanks to its participation in the initiative: International Qualitative Improvement Collaborative (IQIC), led by Boston Children's Hospital. The IQIC database allows you to compare with other programs of centers of high specialty in congenital heart disease worldwide, and also evaluate. Being continuously audited requires the program to demonstrate quality and structure. We also participate in the National Registry of Congenital Heart Disease (RENACAPE) which is a Mexican initiative to carry out a national database. We are part of this project since its inception and this year also, we join as founding members of the International Consortium for the Measurement of Health Results (ICHOM). Although the five affected components form the indispensable platform for a program that can be considered of excellence, the Kardias Program also has: ● Alliances ❏ Texas Children's’s Hospital (TCH): This hospital has remained at the top of the U.S. News & World Report ranking and, as of 2017, controlled the # 1 position in the United States. This alliance allows us to be in contact, learn and share information about cases. The alliance consists of: -Rotations of doctors of Kardias in his Heart Center (+50 rotations with durations of between one week and three months). -Telemedicine sessions: Weekly virtual assistance to the TCH surgical medical session and weekly virtual visit pass of the TCH medical teams with the Kardias team. This strategic alliance has established a close relationship since 2003 with an internationally recognized center for its best practices, great experience and medical talent. ❏ Vall d'Hebron Barcelona Hospital Campus is a world reference health park where assistance, research, teaching and innovation go hand in hand. The alliance consists in opening rotation opportunities for Kardias doctors. Training of specialized medicine cadres ❏ Kardias has a group of specialist doctors who participate in a continuous training program that allows them to be at the forefront in the treatment of congenital heart disease. This program includes participation in national and international conferences of each specialty: surgery, echocardiography, anesthesiology, critical medicine, as well as periodic rotations with partner institutions such as Texas Children's Hospital. ❏ Kardias collaborating doctors, with a high degree of specialization in the different branches of pediatric cardiovascular medicine, permeate their knowledge towards various groups of specialists and medical students. This through teaching, mentoring, teaching seminars and presentations, as well as through papers that have appeared in the most recognized scientific publications, positively impacting the specialization and knowledge of the treatment of congenital heart disease.
