Southcoast VNA is committed to providing high quality comprehensive health and supportive care services in the home setting and community-based programs which foster improvement and maintenance of health, prevention of disease and disability and compassionate end-of-life care.
About This Cause
Southcoast VNA helps people of all ages recover or live with illness or injury through the provision of compassionate, safe and personalized care. We help patients improve function and achieve greater independence, promote optimal well-being and assist with their ability to remain at home. Our services are provided by highly skilled and competent health care professionals and include nursing, rehabilitation therapies, medical social services and home health aides. In addition, advanced specialty services for patients with diabetes, heart disease, chronic obstructive pulmonary disease (COPD), as well as mental health and orthopedic conditions. Supportive Care provides multi-disciplinary care, comfort and support for patients and their families with serious or life-limiting illness.
