About This Cause
Community Health Center is a Federally Qualified nonprofit medical home with services in primary care, behavioral health, dental, optometry, women's health, pharmacy, and nutrition. We serve more than 25,000 patients across Cape Cod and beyond with office locations in Mashpee, Falmouth, Bourne, Sandwich and Centerville. Our vision is to be a health care home of choice reflecting community needs, empowering individuals toward lifelong wellness.
