To advance the independence and full citizenship of individuals with disabilities.
About This Cause
We provide programs for individuals with intellectual and developmental disabilities (I/DD) through state and federal funded programs waivers; school systems and private pay. Day Program locations: Waterford Old Saybrook Groton Stratford Norwich New London Madison East Haddam Colchester Montville Residential: In-home supports. Provide supports that allow the individual to live independently in their own home. Supports are are intermittent based on the needs of the individual. Services include assistance with their personal finances, health and well being, activities that support employment and community inclusion. May include healthcare coordination and transportation. Residential Cluster: In-home supports - Groton. Provide supports that allow the individual to live independently, with additional support overnight. Supports are on a schedule and are intermittent based on the needs of the individual. Supports include assistance with their personal finances, health and well being, activities that support employment and community inclusion. May include healthcare coordination and transportation. Adult Services include: Group Day, individual day and pre-vocational services; in home supports; individualized employment with the goal of attaining competitive employment. Peer Support group, advocacy group and independent living support. Students: Extended Day programs for high school students; Individual day programs and in-home supports for students in their family homes; job-readiness educational supports; internships,life skills training; individualized employment support with the goal of attaining competitive employment. ASSISTIVE TECHNOLOGY LENDING Center: Offering evaluations, assessments and technology technology-based learning aids for students and adaptive and assistive devices for adults to enhance independence with mobility, communication and living skills. Underserved individuals: For under-served individuals with disabilities and their families, we provide information, referral, and service coordination to help them gain access to the resources they need to remove barriers to their independence. People with disabilities, and their families, experience financial hardships and live in poverty at a higher rate than those without a disability. We assist them to prevent homelessness, eviction, and unemployment. Our program will advocate for the individual, assist the individual to find services that provide assistance for heating and energy, transportation, medical equipment, adaptive equipment and assistive technology. We work with community partners and businesses to find affordable and accessible housing and/or provide housing modifications that enable a person or family member to safely access their home. Occasionally, we are able to provide direct financial assistance to avoid “shut offs” and evictions, and then provide follow-up support to ensure future housing stability. Peer Support Group: we offer self-advocacy and workshops to promote independence. At UCP, we treat the person holistically and provide case management to serve them throughout a wide range of service and support needs. Unlike most social service agencies, the Disability Support Program does not require a person to meet a list of criteria and is offered free of charge, regardless of insurance, socioeconomic status, or type of disability.
