Mission Statement
We honor patients and families affected by life-limiting illnesses with integrity, support and compassion. The Connecticut Hospice (CTH) has been a pioneer in compassionate care since the 1970s, when we introduced the hospice concept to the United States. Since 1980, CTH has operated a Home Health Agency, a Hospice Program, and a Short Term Hospital-Special Hospice. Our services recently expanded to include Outpatient Palliative Care services, demonstrating our commitment to evolving with the needs of our community. Our longstanding Magnolia Care component, a specialized hospice care program tailored to the needs of end stage dementia patients, laid the foundation for our successful application to become a Participating Provider for the new CMS Guiding an Improved Dementia Experience (GUIDE) program, called Stand By Me (CTH/SBM) which launched July 1, 2024. The GUIDE program aligns perfectly with our mission, allowing us to expand our reach to a larger and more diverse beneficiary population. By leveraging well-tested navigation and caregiver support strategies, we aim to provide better patient/caregiver experience and support persons living with dementia (PLWD) and their caregivers through the inevitable transitions in care that disease progression demands.
About This Cause
As a not-for-profit, we depend in part upon generous donors who understand that unlike our for-profit competitors, we provide customized services and therapies that are not completely covered by Medicaid, Medicare or private insurance. We strive to go above and beyond the standards set for hospice care. As our logo emphasizes, our goal is to be "always the first, still the best".
