About This Cause
Founded over 30 years ago on the simple premise that all human beings have unsurpassable worth, City Relief provides emergency food relief, access to care, and case management and referral services as an accredited New York City HRA provider and member of the Passaic Continuum of Care (CoC). We help those we serve successfully navigate and overcome barriers to gaining services like shelter, healthcare, permanent housing, and SNAP benefits, as well as access to other material and spiritual resources. City Relief’s outreaches emphasize mobility and flexibility while incorporating a dignity-led approach that deliberately aims to achieve a “multiplier” effect in order to achieve the greatest impact for those who are most in need. All activities and services are provided as part of City Relief’s local and evidence-based informed approach to delivering care. City Relief’s unique mobile model provides holistic wraparound support and professional case management to those struggling with or at risk of homelessness in New York City and New Jersey. We collaborate with state programs, city agencies, and local NGOs to lead the way in innovating new and effective solutions to supporting and reducing the number of unhoused people within our service area. As such, our work serves as the “connective tissue” between the region’s underserved populations and existing resources. City Relief’s approach—in conjunction with our membership to the Rescue Alliance and Homeless Services United (HSU)—enhances, amplifies, and strengthens the sector’s other institutional programs and shelters, and positions us to advocate for and participate in collaborative solutions for widespread impact on homelessness. City Relief’s theory of change is centered around moving those in need toward a pathway of help and hope. We occupy a unique niche within the homelessness services landscape: our innovative, mobile “pop-up” model uses consistent and targeted outreach services five times a week to effectively provide access to essential needs and care in a community-based setting that promotes dignity and trust. This includes integrated case management and referral services, which helps our guests successfully navigate barriers in existing systems. City Relief sets up a mobile outreach using a fleet of buses that are customized with kitchens and resource offices. Our Outreach Team distributes homemade soup, fresh bread, hygiene kits, socks, and other basic necessities to individuals struggling with or at risk of homelessness in New York City and New Jersey, while our trained Follow-Up Care team—many of whom are licensed social workers—provide help accessing housing, Metro Cards, SNAP benefits, Medicare/Medicaid enrollment, detox/rehab programs, and other services. City Relief performs these outreaches eight times per week in five different locations. Every outreach event is joined by one or more Community Partners, who further extend and supplement the services available to our unhoused neighbors. Community Partners are complementary direct service providers that join us on-site at outreaches. They have a wide range of expertise, from legal services, healthcare, and public benefits assistance to COVID-19 testing, housing assistance, and crisis counseling. Having direct contact to these partners helps to minimize obstacles to accessing programs that can be life changing for our clients. After outreaches, City Relief’s Follow-Up Care team works in tandem with our Community Partners to walk alongside our clients and help them through each step of their journey in order to ensure a successful outcome, optimize convenience for clients, and increase the likelihood of continuity of care.
