Mission Statement
Heritage Health and Housing, Inc. is a non-profit 501(c)3 entity in New York City. Heritage has two components to its operations, one is housing which is part of the NYC shelter system and one is a federally qualified health center, which provides medical and dental care to the most medically and economically underserved population in Harlem, NY.
About This Cause
HHCC is a federally qualified health center and part of Heritage Health and Housing, Inc. -a private, 501(c) (3) nonprofit, minority-governed, community-based organization, established in 1968 to serve New York City’s most disadvantage communities. Heritage’s major operations include the development and operation of special needs housing, , and the sponsorship and operation of the Heritage Health Care Center, which is located at 1727 Amsterdam Avenue, New York, NY 10031. HHC currently has 2 School Based Health Centers (SBHC) located at Harlem Village Academy Charter School at 74 W. 124th Street, New York, NY 10027, and at PS 192/Hamilton Grange Middle School at 500 W. 138th Street, New York, NY 10031. Census data collected on our patients reveals that 88% of the health center’s families live at 200% below the federal poverty line.
