We are dedicated to keeping our older neighbors and friends healthy and safe by providing effective programs and supports to help them stay connected to our communities.
About This Cause
The programs offered by LifePath impact most of the social determinants of health and an individual's overall quality of life. Our partnerships with the Cohoes Multi-Service Senior Citizens Center, the Helderberg Senior Center in Berne, the Watervliet Senior Center, and our multiple community dining centers, allow us to serve and support older people adults across Albany County. We serve more than 4,000 older adults annually, some of whom we see nearly every day, others just a few times a year. Most of our service recipients and program participants reside in rural and low-income neighborhoods. The LifePath team comprises 35+ well-trained, professional staff, including certified counselors and a very active and generous Board of Directors. Together, we have the commitment and the capacity to impact the lives of the older adults in our community. LifePath offers programs and services that provide(s): • Senior Center Enrichment - programs and activities including recreation, socialization, and educational opportunities for older adults throughout our region. More than 700 seniors participate in at least one of our Senior Center programs annually. • Nutrition Sites - a hot, nutritious lunch and weekly dinners, often augmented with entertainment. Over 4,000 meals are served collectively at our community dining centers which include the Town of Berne Community and Senior Services Center, Cohoes Multi-Service Senior Citizens Center, Livingston Village Apartments, Ohav Sholom Apartments, South Mall Senior Center, St. Matthew's Church - Voorheesville, Westview Homes, Watervliet Senior Citizens Center, Westerlo Town Hall, Wyman Community Center • Newgate and the Cohoes Companions Social Adult Day Programs - supervision, socialization, and stimulation to older adults who may require extra support and/or companionship during the day and respite to the overwhelmed caregiver. Staffed by caring professionals, we serve 20-30 seniors a day. • Meals on Wheels Program - delicious and nutritious hot and cold meals, prepared in our industrial kitchen located in the south end of Albany. Our meals are made from scratch and provide each recipient with 1/3 to 2/3 of the daily recommended nutritional requirements. This program is more than a meal; each delivery includes a friendly wellness check. We serve nearly 350 older adults daily. • Health and Wellness programs - education and strategies for maintaining optimum health. Classes such as Tai Chi and chair yoga, as well as evidence-based seminars including Aging Mastery Program, FallsTalk, Living With Diabetes, and Patient Activation Measurement (PAM) Program, provide assessment, coaching, and support to improve the overall health and wellness of older individuals. • Community Case Management - one-on-one case management and care coordination for various life transitions such as housing options, retirement, health and wellness, and resources to remain independent and improve one's quality of life. • The Caregiver Connection - educational opportunities and intensive, personalized counseling to caregivers at home or in the workplace, including the Telephone Support Group, which provides caregivers of older adults with information, resources, and a vital connection to other caregivers. • Providing Assistance and Support to Seniors in Transition Program (PASST) - assistance to older adults recently discharged from the hospital to reduce hospital re-admission by providing home-delivered meals, transportation to medical appointments, and case management.
