Mission Statement
NorthPoint Health & Wellness Center (NorthPoint) is community-led, established, and trusted partner to multiple generations of North Minneapolis residents, providing medical, dental, and behavioral health services integrated with access to housing, food, education, employment, and other types of support. Our mission, vision, and values support and celebrate the diverse, caring, and resilient community we serve. Our mission “partnering to create a healthier community” refers both to our clinic/human services partnership as well as to our many longstanding relationships with other community agencies and organizations. Our vision sets a standard of excellence in providing culturally responsive, integrated, holistic primary health and human services that strengthens our community and the lives of the people we serve. We are leaders and partners in a shared vision of a healthy, environmentally safe, and economically stable, self-reliant community.
About This Cause
Programs at NorthPoint meet the presenting, often urgent crisis individuals and families face, like food and housing insecurity, exposure to violence, and mental health crises through an integrated model of health and human services that strengthens our community and the lives of the individuals we serve. Holding true to the community health center model, NorthPoint provides access to medical, dental, behavioral health, pharmacy, optometry, human services, and a community food shelf. Last year, nearly 50,000 community members accessed programs and services at NorthPoint, many of whom live at or below the federal poverty line. We are proud 23,277 individuals engaged in human service programs; which includes 21,597 guests to our food shelf and an additional 2,042 households that participated in housing support and specialty services which includes: 1,201 households who received benefit enrollment assistance (SNAP and MNsure), 301 participants in Project Life a violence prevention program, 228 families participated in Maternal & Family Health programs, 163 households received housing support and case management, 142 men participated in the African American Men Program (AAMP), 69 participants in Career and Workforce Development programs, 51 youth/young adults participated in Youth Services programs and 49 participants in RENEW a domestic violence prevention program. NorthPoint is an established and trusted provider of needed services to multiple generations of North Minneapolis residents. Innovative, whole-person, whole-family, integrated care meets the immediate needs of our community helping them to stabilize, and then addresses the causal factors of instability and provides a path to improved health, wellness, and financial independence.
