LCH's mission is to change lives by serving southern Chester County as the leading provider of integrated health and community services.
About This Cause
Founded in 1973, LCH began as a grassroots organization providing social services to Latinos working as migrant laborers in the mushroom and agricultural industry. Today, LCH offers primary care, women’s health services, pediatrics, dental care, behavioral health counseling, social assistance, outreach, and educational support. LCH’s clients face several barriers to good health, including lack of health coverage, language barriers, poverty, low education, and lack of transportation in this largely rural part of Chester County. LCH’s health services are provided to all clients regardless of insurance status, and no client is ever turned away for inability to pay. In 2012, LCH received designation as a Federally Qualified Health Center (FQHC). LCH is recognized as a Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). We maintain this designation by meeting the five required competencies of comprehensive care, a patient-centered approach, coordinated care, accessibility of services, and quality and safety of care. Our commitment extends to our collaboration with a range of community partners to provide the services our clients need most. LCH has three sites across our service area: our primary care health centers in Kennett Square and Oxford, and our specialty care site in West Grove, encompassing our Women’s Health Center, our Dental Center, the Pia Center for Behavioral Health, and our Pediatric Center. At all three of these sites, our patients can access mental health services and/or social services in tandem with their medical care. Our goal is to treat the whole person, body and mind, to help people access affordable, high-quality care and ultimately to eliminate health disparities and keep the vulnerable patient population we serve in good health. LCH's mission is to improve the health and well-being of people and communities by providing high-quality healthcare, resources, and social services. Together, all of these services are designed to strengthen individual health and resilience; in the aggregate, they make a significant impact on our community as a whole. For the significant percentage of patients who are uninsured or underinsured, LCH is the only place they can receive healthcare; even patients who do have coverage can struggle to access the bilingual, culturally competent care they need. Our community health screenings are another important part of our strategy to ensure access to healthcare. Many individuals in our target population hold two or more jobs which they are juggling with family responsibilities and economic stress. Unless they feel markedly unwell, many will put off routine health care for themselves, even as they make time for their children’s routine pediatric appointments. Our health screenings at local mushroom farms and community centers bring LCH clinical staff to them, taking a range of biometric measures and answering any health questions people may have. We have identified many cases of high blood pressure, elevated HbA1c levels, and mental health distress at these screenings. The work we do to ensure access to care for all in our community is ultimately intended to give lower-income, uninsured, and underinsured residents the same quality care and health opportunities as everyone.
