Esperanza Health Center's Mission Statement is: "Compelled by the love of God in Christ Jesus, in cooperation with the Church and others, Esperanza Health Center is a multi-cultural ministry providing holistic healthcare to the Latino and underserved communities of Philadelphia." We seek to fulfill our mission by providing affordable, high-quality, bilingual and multi-cultural primary health care services in Jesus’ name, regardless of ability to pay, to all those in our communities who seek care.

About This Cause

Founded in 1989 as a small primary care clinic in “El Centro de Oro,” the heart of North Philadelphia’s Latino community, Esperanza Health Center (EHC) is a faith-based federally-qualified health center serving our predominantly Latino and African-American community of nearly 380,000 residents. During 2024, EHC served 14,848 patients through over 66,000 visits. Our three sites are: Fifth Street (2940 N. 5th Street), Hunting Park (4417 N. 6th Street) and Kensington (861 E. Allegheny Avenue). All services at EHC are provided without regard to ability to pay; payment for uninsured patients is on a sliding-fee scale (adjusted annually) based on demonstrated household income, and no one is denied care due to inability to pay. Our holistic care model addresses not only individual health conditions, but also addresses patients’ social, psychological, spiritual and other needs, through an integrated, team-based approach. Our primary care clinicians, including physicians, nurse practitioners, nurse midwives and physician assistants, specialize in family medicine, internal medicine, pediatrics, internal medicine/pediatrics, obstetrics/gynecology and podiatry. Our three dentists and two dental hygienists provide primary dental services (Kensington site). Our behavioral health providers address patients’ mental health and other related concerns. Other integrated services at EHC include: social services/insurance enrollment assistance; medication dispensaries; medication-assisted treatment for OUD/SUD; comprehensive HIV/AIDS medical care; and, inpatient physician care for adult patients at Temple University Hospital. Our Women’s and Prenatal Care team provides OB/GYN care and our Contigo program provides doula care. EHC’s Care Management department serves medically-vulnerable patients with multiple chronic health conditions. Our volunteer chaplains provide spiritual counseling. Because 85% of our patients self-identify as Hispanic/Latino, all clinical staff members speak Spanish. In 2024, EHC received two HRSA Community Health Quality Recognition awards, including the national Health Center Quality Leader-Bronze award (top 21-30% nationally for clinical quality measure performance) for the third year in a row. All three of EHC’s sites continue to have NCQA Patient-Centered Medical Home recognition. Our vision for the next several years is to significantly advance our goals for wellness, to increase wellness and improve health outcomes for EHC's patients and communities. Our Department of Community Health and Wellness collaborates with EHC’s medical and other teams on numerous innovative programs focused on wellness, fitness and health promotion, and which are open to all community members. In 2023, we opened our new community wellness center, The CORE, in Kensington. Over the past year, we have started new group fitness classes such as Zumba and yoga, individual exercise at the CORE Fitness gym, family-oriented programming, classes for our Community Health Promoter program, adult and youth basketball, art and music programs, and more, with many future initiatives planned. Our monthly Hub4Health events offer assistance to community residents, to address food insecurity and to help connect individuals and families with social services and other assistance. Current plans are to develop a new park and recreational space for the entire community. By the end of 2027, we project that we will serve up to 14,000 community residents at The CORE. We will continue to expand access to primary medical care through an expansion of our Fifth Street site, which will add six new exam rooms, group classroom space and a renovated patient waiting area, doubling the size of this site. This expansion is projected to be completed by early 2026.
