About This Cause
TVHC was founded as a health care provider for migrant workers. Today, the agency provides comprehensive medical care, oral health services, vision services, behavioral health services, enabling services, podiatry services, community health education, family support services, and Women Infants and Children (WIC) nutrition to all community members, many of whom are low-income, immigrant, of limited English proficiency, including patients experiencing homelessness or with unstable housing. TVHC provides primary and preventive health care services across the lifespan. Through a combination of direct care and contracted services, a seamless continuum of appropriate care is offered to patients. No patient is denied care because of age, race, gender, religion, sexual orientation or ability to pay. The largest provider of primary care services in Central Alameda County. Founded in 1971, TVHC: • serves as a key part of the safety net for poor, uninsured, immigrant, and vulnerable populations • addresses access to care by serving as a health care home with patient-centered services • saves the health system (and taxpayers) money by preventing hospitalizations, decreasing emergency room use, improving patient health outcomes, and creating good local jobs • has built trust and credibility to address health disparities in the communities we have served for almost 50 years Our patients and clients receive: • A medical home providing integrated primary, dental, behavioral health, and enabling services; there is “no wrong door” to entering our services • Four Community Health Centers including Firehouse Clinic, which is co-located with the Hayward Fire Station • Behavioral health services fully integrated with primary and dental care • Silva Pediatric Clinic • School-based health centers at three high schools in Hayward and Fremont • Wide-ranging community engagement and education and benefits enrollment
