Mission Statement
The mission of Compassionate Home Health Care is to support services and programs for children, the elderly, veterans, those with disabilities and special needs. Compassionate Home Health Care’s mission and purpose is to help provide individuals (seniors, children, Veterans, those with disabilities and special needs) with basic necessities as many people we serve have to decide between living in a healthy, clean space and food. One example of this is: providing laundry detergent and shampoo will ensure less parents have to send their children to school in dirty clothes and without a shower. Objective to achieve with this proposed project/program. Our objective is to help subsidize the high cost of personal care items and cleaning supplies for community members who struggle to purchase basic needs. Compassionate Home Health Care's annual #HolidayGivingCampaign helps provide individuals with basic necessities as many people we serve have to decide between living in a clean space and food. Supplying personal care items will help them with their fixed monthly budgets to their bills and living expenses. Compassionate Home Health Care supports these individuals through its partnerships and friends of the community, Outagamie Housing Authority (River Flat Apartments), ADRC (Aging & Disability Resource Center in Oshkosh, Appleton and Green Bay), St. Vincent de Paul (Neenah, Appleton and Green Bay), NEW Community Shelter, Golden House, Green Bay VA, Center for Veterans Issues, House of Hope, Freedom House, St. John's Homeless Shelter (men’s and womens), Transformation House, Helping Hands Caregivers, Grand View Townhomes, Oneida Heights, Harbor House, Pillars, WIXX Christmas Wish, NAHBRS (Veterans) and local area schools. Many of these community members receive in-home personal and routine home care (assistance with showering, oral care, dressing, cleaning etc.). If these community members do not have shampoo, soap, laundry detergent, toilet paper etc. they go without those services and that authorized caregiver can not bill for the service, in turn they go without pay. We strive to assist in supporting in-home personal care for those in need. Helping those individuals and families start their day feeling and living in a clean space. Each year CHHC’s goal is to reach at least 1000 homes, about 3000 people. This past year with the Pandemic we added our #letsmakeadifference campaign (equal to our annual #HolidayGivingCampaign). In 2020 CHHC was able to reach over 3200 homes, about 9600 people. Between 2011-2016 – Compassionate Home Health Care went from supporting 30/90 homes each year, to reach just over 6500 homes (just over 19,500 people) between 2017-2020. Who will benefit from the project and how? Compassionate Home Health Care supports these individuals through its partnerships and friends of the community, Outagamie Housing Authority, River Flat Apartments, Oneida Heights, Grandview Townhomes. ADRC (Aging & Disability Resource Center, Brown, Outagamie and Winnebago Counties), St. Vincent de Paul, Harbor House, Pillars, WIXX Christmas Wish, NAHBRS (Veterans), St. John's Homeless Shelter for men and women Transformation House, Helping Hands Caregivers, NEW Community Shelter, Golden House, Green Bay VA, Center for Veterans Issues, House of Hope, Freedom House, and local area Schools. By providing personal care/hygiene and cleaning supplies items to individuals in need they are able to feel safe and clean in their living space. Those who receive authorized in-home personal and routine care are able to have the help provided to them with caregivers who assist with their daily living. If these community members do not have shampoo, soap, laundry detergent, toilet paper etc. they go without those services and that authorized caregiver can not bill for the service, in turn they go without pay. What outcomes and measurements will be used to show community impact from the proposed program or project? We serve a wide range of Wisconsin citizens across the state and currently reach people in Brown, Calumet, Outagamie, and Winnebago county. We reach children, men and women. We focus particularly on children, elderly individuals, veterans, those with disabilities or special needs – including special partnerships with local area schools, affordable housing for seniors through Outagamie Housing Authority, Helping Hands Caregivers, St. Vincent de Paul, NAHBRS (for local veterans) and more. All of our partners provide the vetting process, they understand who is most in need. They see the issues that these individuals and family members have everyday to make ends meet. Now more than ever we hear their concern. We are also concerned for them.
About This Cause
Through our “A Community that Care’s” program, we are able to provide opportunities for students at local middle schools (student council programs) and High Schools (DECA and NHS programs) to engage in volunteering, and connect with other community members. Engaging everyone from business owners and senior citizens to students, in order to build "A Community that Cares". Our ultimate goal is to help these individuals and their families have a cleaner, healthier, and happy start to everyday. When you give to Compassionate Home Health Care you're improving lives in our community by, supporting programs run through partner organizations like Outagamie Housing Authority and NAHBRS (for local veterans), Harbour House, NEW Community Shelter, Golden House, Green Bay VA, Center for Veterans Issues, House of Hope, Freedom House, St. Johns Homeless Shelter (men and womens), Helping Hands Caregivers, Grand View Townhomes, Oneida Heights, and many more. Area of impact: Our “A Community that Cares” program brings volunteers together in helping to plan, coordinate, organize, and manage all aspects of each event. Under the management of Compassionate Home Health Care's working board of directors. We offer internships to students from local area middle schools, high schools, technical colleges, and the W2 program. Creating opportunities for individuals to gain knowledge and training in area’s they may find interest in a future career. Helping to build confidence, character, skills, leadership, logistics and an understanding of processes of a campaign, project etc. Compassionate Home Health Care brings wisdom and collaboration, as a tool to help develop self-confidence, team work, leadership, creativite skills to individuals who want to learn, and understand how a campaign, event, grant writing, administrative, event planning, management, web design, marketing, all comes together at any given time of the year. This brings life skills, experience and connections to individuals moving forward in their careers with higher education and more. From beginning stages to completion. Individuals can step into the program at anytime of the year and learn what processes are we are working towards at that time of either our annual #HolidayGivingCampaign and music festival #RocktheDockGreenBay, along with grant writing and funding submissions. With the working board as a team, we are able to work with a variety of individuals aging from 12 and up. Recognizing their gifts and talents and placing them in those areas of the organization to fulfill those particular duties. Learning a skill through leadership and direction The Holiday Season can be stressful for anyone especially those who don’t always have the means for basic personal care/hygiene and cleaning necessities. In order to continue to fulfill our mission Compassionate Home Health Care will continue to support services like personal care/hygiene and routine home care services through our partnerships. For homebound individuals who are unable to do so for themselves and assist with basic essential personal care/hygiene needs and cleaning supplies especially during our annual #HolidayGivingCampaign. This allows individuals to remain independent and stay in their home. Our community volunteers range from grade school and high school students, teachers, missionaries, a roller derby team, local community members all giving their time to help make an impact in our community. Timeline of activities and describe any previous planning. Compassionate Home Health Care’s spring Campaign #letsmakeadiffernce has a spring event to start off the years collections. This campaign runs between February and July. Our #HolidayGivingCampaign events each year during this campaign run October, November and December helping to collect funds and much needed personal care/hygiene and cleaning supplies. As well as our annual #RocktheDockGreenBay (August 29, 2020) music event held on Green Bay Wisconsin's CityDeck. 2019’s event brought about 15,000 attendees, many who brought much needed items for our community members. Due to the Pandemic we moved our in person music event, to virtual. Showcasing many areas of Green Bay, Bay Beach, Hazelwood House, The National Railroad Museum, The Automobile Gallery, The Neville Museum, The Green Bay Botanical Gardens, The Sardine Can and the Stadium View. Along with Rock Garden Studios in Appleton. We pre recorded all of our bands and launched our #RocktheDockGreenBay360 on August 29, 2020. As well as educating and advocating this problem in our community and those individuals who struggle month to month and our solution. Compassionate Home Health Care works with over 150 volunteers each year for our annual #RocktheDockGreenBay music festival (the majority of our volunteers share their time here) and our annual #HolidayGivingCampaign. Roles of collaborating partners. Compassionate Home Health Care seeks the support of the community in the collection of these items for our #HolidayGivingCampaign. We partner with a variety of local business schools between Oshkosh, Appleton and Green Bay ( 60/70 partners a year). Providing them with collection boxes, buckets and posters explaining our partnerships and campaign. We ask our partners to help collect much needed items between October, November and December. Compassionate Home Health Care also provides promotions of the campaign for these local partners through our website, Facebook, Instagram, Twitter pages, press releases and radio partners Woodward Radio Group and Midwest Communications. Compassionate Home Health Care collects the donated items between October thru mid December and holds a bagging event. Inviting partners, local schools and other volunteers to take part in this team building approach. During this bagging event, students not only bag items, they learn the processes of working with a team, leadership, break down, separation and organization of the items to be bagged and distributed. They also engage with local business owners who share their community involvement with our annual events, such as #RocktheDockGreenBay (during this music event, students and interns are able to understand a variety of rolls (leadership roles, event management, management, production etc.) in this area of #Rock the Dock Green Bay (this is part of our "A Community that Care's" program) and #HolidayGivingCampaign. During both the #HolidayGivingCampaign and #RocktheDockGreenBay our collaborating partners assist with collections, donations, volunteers, representatives and leadership. The National Railroad Museum, The Neville Museum, The Automobile Gallery, Helping Hands Caregivers, Xperience Fitness, Copper State/Copper Rock, Turning Point Chiropractic, The Sardine Can, Simmonets, Keggers, Nines, Stirrups, Kittners, Richard Craniums, Bulldogs, Black Saddle, Cropseys, Turning Point Chiropractic, Dairyland Brew Pub, Bagelicous, BioMat, Ahava Salon and Spa, Rejuvenessence Medi Spa, The Bulk Priced-Food Shoppe, The 10th Frame, Stone Toad, Galvans, Copper State and Copper Rock. Compassionate Home Health Care is currently discussing partnerships with the Green Bay Tavern League members. Partnership includes collection of much needed personal care/hygiene and cleaning supplies, volunteers for #HolidayGivingCampaign and #RocktheDockGreenBay, fund-raisers and possible sponsorships. Provide any additional narrative or explanation of budget. Budget revenues are based on actual grants & donations received by the organization the year prior, plus additional grants that have pledged for the current year. Budget expenses are based on actual cash outflow last year plus or minus additional expenditures that are known at the time the budget was created. How will the work be sustained when the proposed funding ends? Our revenue comes from a variety of grants, funding submissions and fundraisers (4). Our #RocktheDockGreenBay music festival held on Green Bay's CityDeck, is our largest fundraiser. Helping to educate, advocate and fund raise for Compassionate Home Health Care's annual #HolidayGivingCampaign. During this campaign we hold 3 smaller fund raisers between October, November and December. Raising funds and collecting much needed items to those community members in need. Grant dollars are used to purchase much needed personal care items (shampoo, soap, toilet paper) and cleaning supplies (laundry detergent, dish soap). Compassionate Home Health Care collects items throughout the year to help those in need. Each year Compassionate Home Health Care seeks the support of the community in the collection of these items for our #HolidayGivingCampaign. The donations received are distributed through our partners (who provide the vetting process) to our local community members in need.
