Mission Statement
The mission of Aging Care Connections is to enrich the quality of life in our community by providing older adults and their families with guidance and support that enhances the ability to age well.
About This Cause
Aging Care Connections was founded in 1971 as the Southwest Suburban Council on Aging and became incorporated as a 501(c)(3) in 1972. In 1983, we established a Care Coordination Unit. In 1989, we were designated as an Elder Abuse Provider Agency. In 2001 we established a formal Caregiver Support program. Our organization underwent a strategic planning process in 2006 to revamp our vision to become a leader in aging-related services in our area. In 2009, we were renamed as Aging Care Connections. We have continued to grow and expand our service area—now encompassing 38 communities in five townships: Lyons, Proviso, Norwood Park, Riverside, and Leyden. In 2015, in partnership with Community Memorial Foundation and Rush Population Health and Aging, we established the Aging Well Neighborhood to create a community-wide approach to aging well. Today, Aging Care Connections serves over 13,000 older adults and families annually, providing comprehensive, client-centered programs that enable older adults to age safely and with dignity in their communities.
