Mission Statement
Motivated by the belief that healthcare is a human right, Erie Family Health Centers provides high-quality, compassionate, and comprehensive healthcare to more than 91,000 patients at 13 health center sites regardless of insurance status, immigration status, or ability to pay. Patients receive integrated medical, dental, and behavioral healthcare services as well as a health education and resources that reduce barriers to health outside the exam room.

Founded on Erie Street in Chicago in 1957, Erie Family Health Centers has grown into a community health center system that provides high-quality, compassionate healthcare services regardless of insurance status, immigration status, or ability to pay. We understand that our communities thrive when every member has the opportunity to live their healthiest life. At Erie’s 13 health centers spanning from the West Side of Chicago to Waukegan, patients receive integrated medical, dental, and behavioral healthcare services under one roof. Recognizing that social, environmental, and behavioral factors account for more than 50% of health, Erie offers programs and resources that reduce barriers to health outside the exam room. At Erie, people of all ages and stages receive holistic healthcare, helping every member of the family stay healthy from infancy through adulthood. Erie Family Health Centers is committed to... ENSURING EQUITABLE ACCESS: We provide affordable primary care for people of all ages, with insurance, without insurance, and regardless of immigration status. We refer patients to specialists and help them obtain access to care via our network of hospital partners. PROMOTING WHOLE-PERSON HEALTH: We offer personalized support between visits including care coordination for chronic disease management, case management for pregnant patients and their babies during the first year of life, and screening for factors affecting health outside the exam room with linkage to essential resources. Patients can participate in a variety of programs to promote health outside the exam room, including a diabetes prevention program, running club, healthy cooking classes, and food distribution events. GROWING THE COMMUNITY WORKFORCE: We address the national shortage of primary care physicians and healthcare workers through training programs including: -seven residencies for physicians; - a fellowship program for advanced practice registered nurses and nurse midwives; and, -pipeline training programs for people seeking to enter health professions, like a medical assistant program offered with NIMAA.
