About This Cause
Hospice is a patient-centered, family oriented approach to care for anyone with a life-limiting illness. Considered to the model for quality, compassionate care at the end-of-life, hospice care involves a team-oriented approach to expert medical care, pain management and emotional and spiritual support expressly tailored to the patient's needs and wishes. Support is extended to the patient's loved ones as well. The core belief of hospice is that each of us has the right to die pain-free and with dignity. Our focus is on caring, not curing. Hospice is provided in a variety of settings including the patient's home, nursing home, and community-based residential facilities. Hospice care services are provided by a team of professionals and trained volunteers, available seven days a week. Services offered for patients and family members include: Physician and nursing care, pain and symptom management, home health aide, social worker, spiritual care counselor, trained volunteers, and respite and bereavement support tp the family while on hospice services and 13 months after the patient's death. People living within approximately a 45-mile radius of Rice Lake are eligible for hospice care. Physicians, hospital social workers, discharge planners or nurses may refer patients to hospice care, but patients may also refer themselves or be referred by friends or family. Hospice services may be covered by Medicaid, Medicare, private health insurance or on a private-pay basis. Providing service are 17 nurses, 7 home health aides, 2 social workers, 2 spiritual care counselors, a massage therapist, physical therapist and physical therapy aides, occupational therapist, speech therapist, 2 volunteer coordinators and 100 hospice volunteers, as well as office support staff. On average we have 50 patients on our caseload.
