About This Cause
Sixteenth Street Community Health Centers (SSCHC) is the largest Federally Qualified Health Center (FQHC) in Wisconsin serving over 43,000 primarily low-income, Hispanic/Latinos in Milwaukee and Waukesha Counties. SSCHC has three, full-service medical and behavioral health clinics in Milwaukee and one in Waukesha. Although we provide critical medical and mental health services for any individual, regardless of ability to pay, SSCHC firmly believes that the lion's share of one's health is impacted by social determinants of health (conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks). At SSCHC, we provide numerous clinic and community-based enabling services that address these social determinants of health (Environmental Health - Lead Outreach and Sustainable Development, Obesity Prevention, Chronic Conditions Health Education - Diabetes and Asthma home visiting, Parenting Resources, Prenatal Case Management, and Social Services). SSCHC also runs one of the largest WIC programs in the state. As a majority of our patients are children, SSCHC has four school-based clinics, and our Layton Clinic location houses one of the only bilingual Child and Adolescent Day Treatment programs in WI.
