About This Cause
Community Health Service Inc. (CHSI) was established in 1973 to provide primary health care services to migrant and seasonal farmworkers and their families. CHSI is a federally qualified health center (FQHC) that provides its services on a sliding fee basis. Care is provided at four year-round clinic locations in Grafton, ND and in Moorhead, Rochester, and Willmar, MN. Mobile units based out of Grafton and Rochester provide medical care within approximately a 75-mile radius of those communities. Victim advocacy staff is located in the Moorhead office as well as a satellite location in Crookston, MN to provide support and advocacy for victims of domestic violence and sexual assault. In an effort to provide holistic, comprehensive care to patients, CHSI has recently expanded services to include onsite behavioral health services, options for telepscyhiatry, and access to suboxone/medication-assisted treatment
