About This Cause

Hospice Care Hospice is end-of-life care that provides comfort and support for those with life-limiting illnesses and their families by attending to their physical, emotional and spiritual needs. The hospice team includes doctors, nurses, certified nursing assistants, social workers, chaplains, volunteers and grief specialists. The Hospice Patient Care is for anyone who has a life expectancy of six months or less based on their physician’s best judgment. This includes, but is not limited to, people with Alzheimer’s or other dementias, heart and liver diseases, cancer, COPD, ALS, Parkinson’s and other conditions related to aging and end of life. The Hospice Team Hospice care is coordinated among team members, the patient and the family. The Hospice team comes to the patient to provide everything they need to be comfortable, thereby allowing the patient to focus on quality time with their loved ones. Medical care – The patient’s comfort is the primary focus of the Hospice medical team, which includes Hospice nurses, our medical director and our pharmacist, all of whom are experts in pain control and symptom management. Equipment, medications and supplies will be managed in the home so the patient experiences fewer trips to the clinic and hospital. The Hospice nurse can serve as the link between the patient and physician to reduce the wait for return calls from the doctor’s office. The nurse will visit as often as necessary to assess medical needs and offer education and support to the caregivers. Personal care – Certified nursing assistants (CNAs) are available to help with the patient’s physical needs, like one-to-one feeding or bathing assistance, shaving and nail care or other personal care services. They can also offer light homemaking services. Emotional support – Social workers are available to help manage any stress the patient and family may be feeling. They are knowledgeable in the emotional aspects of end-of-life care. In addition, they know how to access local and county services and resources for additional support. Spiritual care – Chaplains are professional clergy who are well-versed in the spiritual needs of both patients and families going through the end-of-life experience. They can provide support based on the patient’s personal beliefs and can serve as a link to the patient’s faith community. Assistance with daily tasks – Volunteers can offer companionship by playing cards, reading a newspaper, having coffee, taking walks or whatever the patient finds most comforting. Grief support – Grief specialists offer one-on-one visits, phone calls and literature for up to 13 months, depending on individual needs and comfort level. In addition, they offer uniquely designed groups to provide support and practical suggestions on an ongoing basis during the grief journey.
