About This Cause
Founded in 1996, Health Leads began by connecting people to essential resources like food, housing, and transportation through “help desks” in hospitals and clinics. But addressing individual needs wasn’t enough – lasting change requires transforming systems of health in partnership, including healthcare, public health, and community care. Those who were closest to health challenges are often left out of creating the solutions. Our work today is centered on building a future-focused, resilient, community-led health ecosystem - five to seven years ahead of the current state. We serve as the backbone organization that brings partners together and levels the playing field among health stakeholders to work collectively in the interest of what matters most to communities. We do this by innovating, testing, advancing and spreading racial health equity policies and practices that dismantle systemic barriers to enable people to access the resources they need to live healthy lives.
