Mission Statement
Trenton Health Team’s mission is to improve community health and well-being in the greater Trenton area, New Jersey. We achieve this by partnering with the community to expand access to clinical healthcare and by addressing conditions, like housing quality, food access, neighborhood safety, education and social inequities, that are strongly linked to health outcomes.
About This Cause
Trenton Health Team (THT) plays a vital connecting role between the greater Trenton area and large healthcare and social services systems. Our team of community-facing staff directly serves Trenton residents, connecting individuals with the services they need and helping them advocate for themselves. Other program staff partner with dozens of organizations through stakeholder groups and collaborative projects to understand where individuals' needs are not being met, and what interventions are most needed to achieve positive health outcomes. In turn, THT works closely with State agencies and medical providers, supplying them with the data analysis and insights needed to fill gaps, pilot new interventions and make long term system changes. This work encompasses our role as one of New Jersey’s four State-designated Regional Health Hubs (RHHs), charged with improving Medicaid’s services to its members. Over the last 10 years, THT has pioneered a community care approach that relies on individuals with lived experience in the community to build trusted relationships with residents, in order to support their health and wellbeing. THT team is also the custodian of Medicaid claims data for Mercer County residents, and has become a trusted partner among NJ health-focused institutions, for our ability to bring in-depth knowledge of community together with thoughtful analysis to help improve the systems that serve the community. As one of New Jersey’s four Regional Health Hubs (RHH), THT is a trusted partner of the Department of Human Services. Our staff actively supports NJ FamilyCare/Medicaid priorities, including eligibility redetermination and the expansion of Cover All Kids (CAK). We conduct outreach by phone and in person (in English, Spanish, and other languages via language line), assist community members with Medicaid redetermination paperwork, hold informational sessions, and disseminate vital information through flyers, newsletters, social media, and our website. Governance Established in 2006 as a partnership between Trenton's two acute care hospitals, Capital Health and St. Francis Medical Center; Trenton’s sole Federally Qualified Health Center, Henry J. Austin Health Center; and the City of Trenton's Department of Health and Human Services, THT later incorporated as a 501(c)3 nonprofit in 2010. These founding partners played a leadership role on THT's Board of Trustees for some time. Today, THT operates with an independently appointed Board made up of diverse representatives from partner organizations, funders, and the community we serve. Services Provided At the core of our service is our Community Care model, where CHWs, with lived experience in the greater Trenton community, engage at a personal level with community members and provide support for them to make changes that improve their health, and to access the care they need. For example, CHWs assist with finding housing, transportation or childcare, applying for benefits or accessing care. Beyond our core Community Care program, THT also offers a range of more intense interventions such as housing support, diabetes management, lead abatement and others, that can be provided to clients during their work with their CHW. These specialist interventions are accessed through our Community Care program. In many cases, clients must qualify for these more intense interventions, through diagnosis or circumstance. Our community care work is an integral part of our role as a RHH. In this role we collaborate with dozens of local organizations and conduct data analysis to draw insights that help make an impact on key community indicators.
