About This Cause
Rebirth is a local 501 (c)(3) nonprofit community based organization locates in Salisbury, Wicomico County, the Eastern rural region of MD. It has been providing several essentials programs to the low-income, immigrants, families and workers for over 25 years. Our Mission is to help advocate for rights and help facilitate resources to the underserved, low income immigrant families in our community. We directly assist low-income, uninsured, immigrant or underinsured individuals to gain access to free or reduced-price clinical healthcare services through, Coordination of patients' health care. We connect patients with appropriate providers, navigate appointment scheduling, and ensure continuity of care. We collaborate with community organizations to identify the cultural needs and preferences of the communities we serve and be a bridge. This helps ensure that we exchange, share our resources and services are culturally appropriate and meet the needs of our clients.We provide Culturally Appropriate Health Education, literacy, prevention and outreach to the immigrant community. Train workers and promote Health & Safety in immigrant populations is difficult due to cultural, linguistic and socio-economic barriers. These services include: Healthcare For The Rest Of Us Healthcare for the rest of us is an initiative gears toward the immigrants and refugees population, who have no access to healthcare, medication, transportation and more. It brings healthcare access to the newly arrived immigrant with no insurance or money, by bringing a team of healthcare professionals from the local hospital to our centers every other Tuesday to see new patients. It provides accessible and culturally sensitive care through partnerships with local healthcare institutions/professionals and community outreach. Case management services: We provide dedicated support to patients, addressing social determinants of health, and assisting with referrals for additional services. Our programs/service includes Food, Health Education & outreach, Translating & Interpreting, After school, summer camp, Women & babies closet community, Annual Events: Baby Shower, Back Pack Giveaway: These services include: 1. Employment Services: Assisting clients in securing gainful employment through job readiness training, resume building, interview preparation, and job placement assistance. 2. Transitional Financial Assistance: Providing temporary financial support to alleviate immediate needs such as housing, utilities, transportation, and other essential expenses during the transition period. 3. Health Assistance: Facilitating access to healthcare services, including medical screenings, vaccinations, mental health counseling, and referrals to specialized care providers. 4. English Language and Vocational Training: Offering English language classes tailored to the needs of adult learners, as well as vocational training programs to enhance employability and career prospects. 5. Case Management: Providing personalized case management services to assess individual needs, develop action plans, and navigate available resources and support networks. 6. Youth Programming: Offering culturally sensitive youth programs and activities to promote academic success, social integration, and leadership development among young refugees and asylees. 7. Community LPFM Radio Station. Transportation of patients to and from medical appointments: We offer transportation assistance to remove barriers and ensure patients can attend crucial appointments. These services include: Navigation Services Care management support, connecting people with community resources, and social service needs. Linking patients and family members with essential health needs, such as: assist in improving physical, mental, social health, facilitate healthy lifestyle programming, screen, and address SDOH among program participants. We also help them find affordable housing, food, clothing, transportation, school registration, health insurance and more. Conclusion We are confident that our organization aligns with your criteria.
