About This Cause
The Planning Council serves as a regional resource by helping communities throughout Hampton Roads and beyond, implement effective strategies for those issues that cut across jurisdictional boundaries. We administer a variety of local, state, and federal programs bringing the highest quality of care to individuals experiencing homelessness, to military families with a special needs child, and supporting in-home and daycare centers, including serving nutritious food to children in their care. Additionally, we advocate for initiatives that benefit the region. COC COORDINATION TEAM The Coordination Team provides comprehensive facilitation of the homeless services Continuums of Care (COC) that include ~130 partner agencies across Greater Hampton Roads, coordinating annual applications for $12M in homeless services and management of processes to end homelessness. HMIS TEAM The Homeless Management Information System Team oversees the database used by ~70 partner organizations who serve individuals and families experiencing or at risk of homelessness in the Hampton Roads and Central Virginia communities. The aggregate data demonstrates the trends and demographics of those receiving homeless-related services, which is then reported to Congress to help guide further resources from both federal and state agencies. THE NAVY RESPITE PROGRAM The Respite Program assists ~400 Navy families who have an Exceptional Family Member (child with special needs) by offering respite care services. We support ~200 skilled childcare providers with relevant training and monitoring. We ensure they serve each family well. CHILD & ADULT CARE FOOD PROGRAM CACFP is a USDA program that reimburses childcare providers (in-home and centers) at $1 million for 548,000 nutritious meals and snacks served to 14,000 children each month in their care. We monitor ~1400 sites in MD, DC, and Eastern VA. NEEDS ASSESSMENT & COMMUNITY PLANNING TPC conducts a variety of needs assessments, which provide an in-depth understanding of the most important needs for the community and guides future action. Our Planning team analyzes resources, capacity, and gaps in services, identifies the most vital needs and prioritizes the changes needed. In the past fifteen years, TPC has conducted 9 community health needs assessments for health departments and hospitals in Hampton Roads, the Eastern Shore, and Central Virginia. Of these, the largest community needs are access to specialty health services, behavioral health, transportation, affordable medications, and employment. EMPLOYER FINANCIAL ASSITANCE FUND ADIMINSTRATION The Planning Council partners with Sentara Healthcare, Eastern Virginia Medical School, and City of Chesapeake to administer their employee assistance programs, which expends over $70, 000 per month in financial assistance to their employees. The TPC team offers case management to over 755 employees per month that find themselves in a crisis.
